摘要：消化道肿瘤发病率较高,对人们的健康造成了严重威胁,其早期临床症状并不典型且诊断率低,患者出现明显临床症状时多数已发展为进展期或晚期,此时即使进行手术,手术效果及预后均不理想。早期消化道肿瘤的治疗主要以手术根治为主,而进展期胃癌还需要联合放化疗等方式进行后续治疗,但由于化疗药物多耐药或者放疗患者不耐受等原因,导致患者预后不佳。在这种情况下,术中放疗(IORT)被认为是一种潜在的、有价值的治疗选择。IORT可被定义为在手术过程中对肿瘤床直接施加高辐射剂量。这种方法能精确地使大量的、单一部分的辐射直接且特殊地输送到局部复发风险高的解剖焦点区域,而辐射剂量及其对周围正常组织的相关毒性可以通过将其从辐射区域移开或屏蔽来限制。因此,明确IORT的作用原理及机制对消化道肿瘤的治疗及预后有着十分重要的作用。
The incidence of gastrointestinal tumors is high,which poses a serious threat to people s health.Their early clinical symptoms are not typical and the diagnosis rate is low.Most patients have developed advanced stages when they have obvious clinical symptoms.Even if the surgery is performed at this time,the surgical effect and prognosis are not ideal.The treatment of early gastrointestinal tumors is mainly based on radical surgery,while advanced gastric cancer needs to be combined with radiotherapy and chemotherapy for follow-up treatment.However,the prognosis is poor due to multiple drug resistance to chemotherapy drugs or patient s intolerance to radiotherapy.In this case,intraoperative radiotherapy(IORT)is considered a potentially valuable treatment option.IORT can be defined as the direct application of high radiation doses to the tumor bed during surgery.This method can accurately deliver a large amount of single part of radiation directly and specifically to the anatomical focus area with high risk of local recurrence,and the radiation dose and its related toxicity to surrounding normal tissues can be limited by removing or shielding them from the radiation area.Therefore,clarifying the principle and mechanism of IORT plays a very important role in the treatment and prognosis of gastrointestinal tumors.
