摘要：肛瘘是肛肠科常见病之一,临床以局部反复流脓、疼痛、瘙痒为特点,并可触及条索样结节。由于其难以自愈、易反复,故常采用手术治疗,术后常常伴随疼痛等并发症,影响患者的生活质量。中医药具有活血化瘀、清热燥湿、消肿止痛等功效,临床上常采用中药治疗术后并发症,而且效果显著,越来越受到广大医生和患者的认可。现代研究发现,中药对肠道菌群的紊乱有一定的调节作用,又有研究表明肠道菌群紊乱与肛肠疾病术后并发症发生密切相关。中医学认为术后的主要病机为湿热下注。湿热证与肠道菌群紊乱密切相关,因此,临床常用清热利湿药来调节肠道菌群。推断清热利湿药可能是通过调节肠道菌群以控制炎症因子的表达,进而发挥其治疗肛瘘术后并发症的作用。文章从肠道菌群出发,阐述清热利湿药与肛瘘术后并发症的关系,期望为中医药治疗肛瘘术后并发症的作用机制提供依据。
Anal fistula is one of the common diseases in the department of anorectal.Clinically,it is characterized by local recurrent pus,pain and itching,and cord-like tubercles can be touched.Due to its difficulty in self-healing and recurrence,surgical treatment is often used.Postoperative complications such as pain often affect the quality of life of patients.TCM medicine has the effects of promoting blood circulation and removing blood stasis,clearing heat,dryness and dampness,reducing swelling and relieving pain.TCM medicine is often used in clinical treatment of postoperative complications,and the effect is remarkable,more and more recognized by the majority of doctors and patients.Modern studies have found that TCM medicine can regulate the disorder of intestinal flora,while other studies have shown that intestinal flora disorder is closely related to postoperative complications of anorectal diseases.TCM believes that the main pathogenesis of post-operative anal fistula is dampness-heat in lower-Jiao(下焦).Damp-heat syndrome is closely related to the disorder of intestinal flora,so clearing heat and promoting diuresis drugs is commonly used to regulate intestinal flora in clinic.It is concluded that clearing heat and promoting diuresis drugs may play their role in treating postoperative complications of anal fistula by regulating intestinal flora in order to control the expression of inflammatory factors.Therefore,this paper,starting from intestinal flora,elaborated the relationship between heat-clearing and damp-removing drugs and postoperative complications of anal fistula,hoping to provide a basis for the mechanism of action of TCM medicine in treating postoperative complications of anal fistula.
