摘要:骨质疏松症是骨骼系统以骨量下降、骨强度降低,骨微观结构发生破坏为主的慢性代谢性疾病,是骨内科学系统的一种常见病、多发病,严重威胁老年人群骨骼健康。骨质疏松症发病的主要机制是骨骼微循环代谢紊乱后,导致成骨细胞与破骨细胞的动态性生理平衡被打破。研究已证实磷脂酰肌醇-3-激酶(PI3K)-丝氨酸/蛋白激酶B(Akt)信号转导系统是维持骨骼内环境正常代谢的重要关键靶向通路,在维持骨骼血液循环稳态方面具有关键作用,PI3K/Akt信号通路与骨骼血液循环方面的相关机制已经成为现代医学研究的热点。近年来,有关中医药分子靶向药物与PI3K/Akt信号通路的治疗相关性的分子生物学免疫研究不断深入,且在骨质疏松症的治疗中取得良好疗效。中医学认为骨质疏松症的发生机制以肾虚血瘀为主,基于此病机,补肾活血法在治疗骨质疏松症中应用广泛且临床疗效显著。文章将从PI3K/Akt信号通路作用机制、补肾活血法的干预效应及骨质疏松症的分子生物学免疫对接靶点等方面展开论述。
Osteoporosis is a chronic metabolic disease of the skeletal system characterized by decreased bone mass,bone strength and damage to bone microstructure.It is a common disease with frequently occurring in the bone internal medicine system,which seriously threatens the bone health of the elderly.The main mechanism of osteoporosis is the broken dynamic physiological balance between osteoblasts and osteoclasts due to the disturbance of bone microcirculation metabolism.Studies have confirmed that phosphatidylinositol-3-kinase(PI3K)-Serine/protein kinase B(Akt)signal transduction system is an important and key targeted pathway to maintain normal metabolism of the skeletal environment,and plays a key role in maintaining skeletal blood homeostasis.The related mechanism of PI3K/Akt signaling pathway and bone blood circulation has become a hot topic in modern medical research.In recent years,molecular immunological studies on the therapeutic correlation between molecular targeted drugs of traditional Chinese medicine and PI3K/Akt signaling pathway have been deepening,and good efficacy has been achieved in the treatment of osteoporosis.In ancient Chinese medicine,the main mechanism of osteoporosis is kidney deficiency and blood stasis.Based on this pathogenesis,tonifying kidney and activating blood circulation method has been widely used in the treatment of osteoporosis with remarkable clinical efficacy.This paper will discuss the mechanism of PI3K/Akt signaling pathway,the intervention effect of tonifying kidney and activating blood circulation method,and the molecular biological immunetarget of osteoporosis.
