摘要:目的探讨及评价模拟住院医师教学法在外科实习教学中的应用价值。方法将102名本科实习生按照人科的不同月份,分为实验组及对照组;对照组52人参与正常临床实习工作,实验组50人同时扮演“实习生”及“住院医师”双重角色,在教师指导下模拟住院医师进行工作,对自己所管患者独立问病史、体格检查、自主分析、给出合理诊断及治疗方案,建立独立的诊疗病历档案。实习结束后对两组学生进行同样的出科综合技能考核,并对实验组实习生进行问卷调查。结果实验组综合技能评价总分及各项目评分均高于对照组,差异有统计学意义。96.0%的实习生认为模拟住院医师教学法有利于提高学生实践创新能力,90.0%的实习生认为该法有利于独立思维能力的培养,100%学生认为该法对主动学习能力的培养有较大的帮助。结论模拟住院医师教学法可以提高实习生综合实践技能,提高实习教学质量,使学生更快适应临床工作,无明显临床安全隐患,可在临床实习中加以应用。
Objective To observe and evaluation the value of simulating residents teaching method in surgical practice. Methods A total of 102 undergraduate interns were divided into experimental group and control group to clinical practice according to different months. The interns of control group (50 persons) participate in normal clinical practice,experimental group play the dual role as a "trainee" and "residents" to work, who are required to ask patients' history independently, to communicate with patients, to take physical examination, to analyze problems, to make a reasonable program of diagnosis and treatment and to establish an independent clinic medical record. The same evaluation of comprehensive skill was made between the two groups and a questionnaire was conducted in test group after the practice. Results The total and item scores of comprehensive assessment of experimental group were higher than that of control group, the difference was statistically significant, The interns of 96.0% of experimental group deemed that the teaching methodology of simulating resident to practice would improve students'practical innovation ability, those of 90.0% thought that this way was advantageous to improve independent thinking ability and 100% of which indicated it had great help to the active learning ability's raise. Conclusions The simulating residents teaching method is helpful to improve the comprehensive practice skills of the students and enhance the teaching quality. There are a great prospect of popularization and application in clinical practiceand no significant clinical safety problems.
