摘要:目的分析兰州市西固区体检人群的幽门螺杆菌(Helicobacter pylori,Hp)感染分布特征及相关危险因素,为预防Hp感染提供参考依据。方法选取对象为2019年1月—12月兰州市某医院的8118例体检人员,Hp感染检测用14C尿素呼气试验,通过自制问卷收集体检者年龄、性别、吸烟、饮酒、职业类别、体质指数(body mass index,BMI)、饮食偏好、饮食口味、血脂等资料,分析这些因素与Hp感染的关系。结果8118例研究对象,Hp阳性4244例,检出率52.28%。男性Hp检出率53.16%,女性Hp检出率50.27%,差异有统计学意义(P<0.05)。单因素分析显示,高龄、吸烟、饮酒、长期服药、饮食口味(辛辣刺激、高盐高脂)和饮食偏好对Hp感染率显著影响(P<0.05);logistic回归分析显示,高龄、吸烟、高盐高脂、辛辣刺激饮食、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、Hcy、GLU异常,为影响Hp感染的独立危险因素(P<0.05)。结论兰州市西固区体检人群Hp感染率较高,其影响因素多方面,随年龄增加呈升高趋势,吸烟、高血糖、饮食口味与Hp感染显著相关,应根据相关危险因素,对体检人群进行针对性管理。
Objective To analyze the distribution characteristics and related risk factors of Hp infection in the physical examination population in Xigu District of Lanzhou City,and to provide a reference basis for the prevention of Hp infection.Methods The selected subjects were 8118 physical examination personnel from a hospital in Lanzhou from January to December 2019,Hp infection was detected by 14C urea breath test.The self-designed questionnaire was used to collect the age,gender,smoking,drinking,occupation category,body mass index(BMI),dietary preference,dietary taste,blood lipid and other data of the physical examination participants to analyze the relationship between these factors and Hp infection.Results Of the 8118 study subjects,4244 were Hp positive,with a detection rate of 52.28%.The Hp detection rate for males was 53.16%,and for females was 50.27%,with a statistically significant difference(P<0.05).Univariate analysis demonstrated that advanced age,smoking,alcohol consumption,long-term medication,dietary taste(spicy and stimulating,high-salt and high-fat)and dietary preferences had significant effects on the prevalence of Hp infection(P<0.05).Logistic regression analysis showed that advanced age,smoking,highsalt and high-fat diet,spicy and stimulating diet,the abnormalities of high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),Hcy and GLU were independent risk factors of Hp infection(P<0.05).Conclusion The prevalence of Hp infection is high in the physical examination population in Xigu District,Lanzhou City,and the influencing factors are multifaceted and increased with age.Smoking,high blood sugar and dietary tastes are significantly associated with Hp infection,and the targeted management should be performed for the physical examination population according to the relevant risk factors.
