摘要:目的研究第一跖骨头下籽骨与踇趾外翻的相关性。方法对2000例正常足及60例踇外翻足分别进行下列测量:踇外翻角(HVA)、第1、2跖骨间角(IMA)、第二跖骨长度、胫侧籽骨与第一跖骨中线长轴相交值、腓侧籽骨与第一跖骨外侧皮质相切值、胫侧籽骨至第一跖骨中线长轴距离、腓侧籽骨至第一跖骨中线长轴距离及胫侧籽骨与腓侧籽骨距离并统计分析。结果正常足组中腓侧籽骨与第一跖骨外侧皮质相切值、胫侧籽骨与腓侧籽骨距离、胫侧籽骨至第一跖骨中线长轴距离与第二跖骨长度的比值与HVA有相关性(P<0.05),而胫侧籽骨与第一跖骨中线长轴相交值与HVA无相关性(P>0.05),IMA只与胫侧籽骨至第一跖骨中线长轴距离与第二跖骨长度的比值有相关性(T=-3.37、P=0.001)。踇外翻足组中,胫侧籽骨与第一跖骨中线长轴相交值、腓侧籽骨与第一跖骨外侧皮质相切值均与HVA有高度相关性且与籽骨与第一跖骨中线长轴相交值呈正相关、与腓侧籽骨与第一跖骨外侧皮质相切值呈负相关(P<0.05、OR 0.804)。结论第一跖骨头下籽骨脱位是导致踇趾外翻的一个重要病理因素,且胫侧籽骨向外脱位越明显HVA越大,腓侧籽骨与第一跖骨外侧皮质相切越小、HVA越大。
Objective To study the correlation between the first metatarsal head sesamoid and the metatarsophalangeal valgus.Methods The following measurements were made in 2000 cases of normal foot and 60 cases of metatarsus valgus:Hallux valgus(HVA),first and second metatarsal angle(IMA),second metatarsal length(CD),tibial segment.The distance from the long axis of the first metatarsus,the tangent value of the lateral cortex to the lateral side of the first metatarsus,the distance from the tibial cortical bone to the longitudinal axis of the first metatarsus,The distance from the sesamoid was statistically analyzed.Results The tangential value of the lateral cortical bone in the normal foot and the lateral arc of the first metatarsus,the length of the second metatarsus,the distance between the tibial segment and the fibular seamoid bone,the distance between the tibial sesamoi bone and the first metatarsal midline,IMA was correlated and positively correlated(P<0.05),but there was no correlation between the sesamoid bone and the long axis of the first metatarsal bone and HVA and IMA(P>0.05).The tangential values of the lateral cortical bone and the lateral metatarsal bone were correlated with HVA and IMA,and the relationship between the lateral sarcoplasmic reticulum and the first metatarsal(P<0.05,OR 0.804)was positively correlated with the lateral cortical tangent of the first metatarsal bone.Conclusion Segmental dislocation of the first metatarsal head is an important pathological factor leading to valgus,and the more obvious the dislocation of the tibial lateral sesamoid bone is HVA,the larger the IMA is,the fibular sesamoid bone is tangent to the lateral cortex of the first metatarsa.The smaller the HVA,the larger the IMA.
