摘要：干眼是临床中常见的慢性眼表疾病,近年来发病率不断上升并趋于年轻化态势。最新的中国干眼专家共识肯定了“眼表微环境失衡”在干眼发病中的主要作用。近年的研究表明,肠道菌群对眼表微环境具有调节作用,并参与干眼的发生发展。中医理论认为,肠道菌群是中医脾藏象理论的现代体现,众多医家从脾论治干眼取得了良好的治疗效果。本文以肠道菌群为基点,通过阐述肠道菌群与干眼的关系、脾与肠道菌群的联系、脾胃调节中药对干眼患者及肠道菌群的影响,提出“肠道菌群?脾?眼表微环境”的作用形式,揭示了脾胃调节中药通过调节肠道菌群,恢复干眼患者眼表微环境而发挥治疗作用,为从脾论治干眼提供了现代理论依据。
Dry eye is a common chronic ocular surface disease,the incidence of which has been increasing and tends to be higher in younger population in recent years.The latest Chinese expert consensus on dry eye affirms the importance of the"imbalance of the ocular surface microenvironment"in the pathogenesis of dry eye.Recent studies have shown that intest-inal flora has a regulatory effect on the ocular surface microenvironment and is involved in the development of dry eye.Ac-cording to the theory of traditional Chinese medicine(TCM),intestinal flora is the modern presentation of TCM theory of"visceral manifestation of the spleen",and many doctors have achieved good therapeutic effects in treating dry eye from the theory of spleen.Therefore,with intestinal flora as the basis point,and through elaborating the relationship between intest-inal flora and dry eye,the connection between spleen and intestinal flora,and the influence of TCM drugs regulating spleen and stomach on patients with dry eye and their intestinal flora,this paper proposed the form of the action of"intestinal flora-spleen-ocular surface microenvironment",and revealed that TCM drugs regulating spleen and stomach play a therapeutic role by adjusting the microecology of intestines and restoring the ocular surface microenvironment of patients with dry eye,which provides a modern theoretical basis for the treatment of dry eye from the perspective of the spleen.
