摘要：目的:探讨疏风清热汤加减治疗急性扁桃体炎的临床经验。方法:回顾性分析甘肃中医药大学附属医院耳鼻咽喉头颈外科门诊2023年11月―2024年1月收治的2例急性扁桃体炎患者的临床资料,并对相关文献进行复习。结果:患者临床均见痛苦面容,自述咽痛,吞咽痛甚,伴或不伴发热,舌质红,舌苔黄,脉象数脉。间接喉镜示:腭扁桃体Ⅰ―Ⅱ度肿大,表面兼见少量脓性分泌物,咽峡充血红肿。本病属中医学“急乳蛾”范畴,初期主要为风热之邪外袭咽喉,因病位在上,故治法应以清为主,临证予以疏风清热汤加减,以疏风清热、利咽消肿。经治疗,2例患者咽痛症状明显减轻,发热未反复,间接喉镜示扁桃体肿胀消减,脓性分泌物未见,咽峡稍红,镜下未见明显充血红肿,临床症状缓解明显。结论:疏风清热汤加减治疗急性扁桃体炎具有确切临床疗效。西医学治疗本病多采用抗生素及手术切除两种方式。中医辨证施治,具有风险小、价格实惠且不易复发等优势,同时,现代药理研究证实,疏风清热汤具有抗菌、镇痛的作用,临证使用时不良反应较少,值得临床应用和推广。
Objective:To explore the clinical experience of the modified Shufeng Qingre decoction(疏风清热汤)in Othe treatment of acute tonsillitis.Methods:The clinical data of two patients with acute tonsillitis in the Department of Otolaryngology,Head and Neck Surgery,Affiliated Hospital of Gansu University of Chinese Medicine from November 2023 to January 2024 were retrospectively analyzed,and the related literature was reviewed.Results:The clinical manifestations of two patients included painful complexion,self-reported pharyngeal pain,severe swallowing pain,with or without fever,red tongue,yellow tongue coating,and rapid pulse.Indirect laryngoscopy showed that the palatine tonsils were enlarged in I to II degrees,with a small amount of purulent secretions on the surface and hyperemia and swelling of the pharynx.This disease belongs to the category of“acute nipple moth”in traditional Chinese medicine,the main is the evil of wind and heat attacking the throat,because the disease is on the top,the treatment should be based on clearing method,and clinically the modified Shufeng Qingre decoction is given,which has the effect of dissipating wind,clearing heat,soothing throat and reducing swelling.After treatment,the symptoms of pharyngodynia in two patients were significantly alleviated,fever did not recur,the tonsil swelling was reduced compared with the previous indirect laryngoscope image,no purulent secretions were observed,the pharyngeal isthmus was slightly red,there was no obvious congestion and swelling under the microscope,and the clinical symptoms were significantly relieved.Conclusion:The modified Shufeng Qingre decoction has definite clinical effect in treating acute tonsillitis.Western medicine mostly uses antibiotics and surgical excision to treat this disease.TCM treatment based on syndrome differentiation has the advantages of low risk,affordable price and being not easy to relapse.At the same time,modern pharmacological research has confirmed that the Shufeng Qingre decoction has antibacterial and analgesic effects,and the occurrence of adverse reactions in clinical application is less,which is worthy of clinical application and promotion.
