摘要：肛周脓肿是由多菌种混合感染引发的肛肠急症,以起病急、疼痛剧烈、发展迅速为主要特征,临床以手术切开引流作为主要治疗手段,其术后开放性创面因解剖位置特殊易继发感染,影响愈合进程。中医外治法基于“祛腐生肌”理论,通过中药外敷、熏洗、药线引流等途径促进肛周脓肿术后创面愈合,疗效显著。通过整理近年来中医药外治法促进肛周脓肿术后创面愈合的相关临床研究进展,并分析其作用机制,为肛周脓肿术后精准治疗提供理论支撑。
Perianal abscess is an acute anorectal emergency triggered by polymicrobial infection,characterized by abrupt onset,excruciating pain,and rapid progression.Surgical incision and drainage remains the gold-standard treatment;however,the resulting open wound—situated in a contaminated anatomical milieu—is exceptionally prone to secondary infection,thereby impeding the healing trajectory.Guided by the traditional Chinese medicine(TCM)paradigm of“eliminating necrotic tissue to generate new flesh,”external TCM interventions—including topical herbal poultices,medicated sitz-bath fumigation,and herbal-thread drainage—have demonstrated pronounced efficacy in accelerating postoperative wound healing.This paper synthesizes contemporary clinical advances in the application of external TCM modalities for perianal abscess wounds and delineates their mechanistic underpinnings,thereby furnishing an evidence-based theoretical framework for precision management in the postoperative phase.
