摘要:目的 研究早期Twin-Block(TB)功能矫治器矫治对安氏Ⅱ~1类错(牙合)畸形患儿软、硬组织的影响。方法 对42例安氏Ⅱ~1类错(牙合)畸形患儿进行早期TB功能矫治器矫治,通过X线正中(牙合)位头颅侧位定位片分析功能矫治前、功能矫治后患儿软组织和硬组织相关测量指标的变化。结果 早期TB功能矫治器矫治能够促进安氏Ⅱ~1类错(牙合)畸形患儿下颌骨的生长发育,使下颌基骨长度、下颌升支高度明显增加(P<0.05),上、下颌骨矢状向关系得到改善,同时发生以下颌磨牙区牙槽骨高度增加为主的牙槽骨代偿性改建;还可使软组织上唇突距明显缩小,上唇基部厚度、上唇紧张度、下唇突距、颏突度、软组织面突角明显增大(P<0.05或P<0.01)。结论 对安氏Ⅱ~1类错(牙合)畸形患儿进行早期TB功能矫治器矫治,可改变硬组织颌骨间关系,使软组织颏部突出,改善鼻、唇、颏软组织形态结构,使患儿拥有良好的侧貌面型。
Objective To study the effects of early orthodontic treatment with the Twin?Block(TB)appliance on soft and hard tissues in children with Angle’s classⅡ1 malocclusion.Methods Forty?two children with Angle’s classⅡ1 malocclusion were performed with early orthodontic treatment with the TB appliance,and the changes of soft and hard tissue related measurement indexes before and after early orthodontic treatment with TB appliance were analyzed by lateral jaw X?ray in the centric occlusion position.Results Early orthodontic treatment with TB appliance could promote the growth and development of mandible in children with Angle’s classⅡ1 malocclusion,significantly increase the length of mandibular basal bone and the height of anterior portion of mandibular ramus(P<0.05),and the sagittal relationship between the maxilla and mandible was improved,meanwhile,the alveolar bone compensatory remodeling occurred was mainly caused by the increase of alveolar bone height in the mandibular molar area.It could also significantly reduce the distance of the upper lip protrusion of the soft tissue,and signifi?cantly increase the thickness of the base of the upper lip,the tension of the upper lip,the distance of the lower lip protrusion,chin protrusion and the protrusion angle of the soft tissue surface(P<0.05 or P<0.01).Conclusion Early orthodontic treatment with the TB appliance on children with Angle’s classⅡ1 malocclusion can change the relationship between hard tissues in mandibula,make the soft tissues of chin protrude,improve the soft tissues mor?phology and structure of nose,lip and chin,and make the children have good lateral profiles of the face.
