摘要:目的观察胡蓝降糖缓释片对糖尿病肾病(DN)模型大鼠的治疗作用,检测其对肾脏功能、细胞中蛋白激酶A(PKA)活性以及细胞外基质(extracellular matrix,ECM)的影响,探讨胡蓝降糖缓释片对DN大鼠肾脏治疗作用的机制。方法采用单侧肾脏切除联合腹腔注射链脲佐菌素(STZ)的方法制备DN大鼠模型;随机分正常对照组、模型组、艾塞那肽组及胡蓝降糖缓释片(TEG)组,分别给予相应药物治疗,6 w后检测各组大鼠空腹血糖(FBG)、24 h尿微量白蛋白(24h-m ALB)及大鼠肾皮质细胞中蛋白激酶A(PKA)活性,并进行肾组织形态学观察。结果给药6 w后,与模型组比较,胡蓝降糖缓释片组FBG(t=2.837)、24h-m ALB水平明显降低(t=4.531),抑制了PKA的活化度(t=2.800),肾脏功能得到一定程度的恢复(均P<0.05)。结论胡蓝降糖缓释片通过抑制PKA活化表达度,调节ECM分子物质生成链,改善了DN中ECM的病理变化程度。
Objective To observe effect of Hulan hypoglycemic sustained-release tablets treating for the rat model suffer from diabetic nephropathy(DN), testing its effect on kidney function, protein kinase A(PKA) activity in cells and extracellular matrix(ECM), and investigate mechanism of Hulan hypoglycemic sustained-release tablets treating for the kidney of rat model. Methods Unilateral nephrectomy and intraperitoneal injection of streptozotocin(STZ) were taken to prepare the DN rat model, and to divide into normal control group, model group, exenatide and Hulan hypoglycemic sustained-release tablets(TEG) group randomly, and give the appropriate medication, then to detect the fasting blood glucose(FBG), 24h-microal buminuria(24h-m ALB) and the PKA activity in renal cortex cells of rat after 6 weeks, and to give renal tissue morphology. Results Compared with the model group, the fasting blood glucose(FBG, t=2.837) and the 24h-microal buminuria(24h-m ALB, t=4.531) of Hulan hypoglycemic sustained-release tablets(TEG) group decreased obviously(P〈0.05), the Hulan sustained-release tablets inhibited the activation of the PKA(t=2.80,P〈0.05), and restored the kidney function partly.Conclusions Hulan hypoglycemic sustained-release improves the pathological changes of the extracellular matrix(ECM)in diabetic nephropathy(DN) of rat model by inhibiting the expression and activation of the PKA and regulating the substances generated chain of extracellular matrix(ECM) molecules.
