摘要：为了指导中医药学对动脉粥样硬化症(AS)的临床应用和研究。分析和归纳脾胃学说的中医典籍,提出脾胃虚弱升降失常是AS的核心病机、调补脾胃是防治AS的关键的学术观点。以胸痹为例,在中医药综合数据库(TCMID)中检索中医典籍记载治疗胸痹的方剂,并对方剂进行结构化处理和统计分析;总结中医药通过调补脾胃治疗脉粥样硬化症的基础研究进展。结果表明,治疗AS的方剂大多含有调补脾胃之药,有温中祛寒、补气健脾之效;中药通过抑制内皮细胞系统(ECs)损伤和保护ECs,调节血管舒缩抑制血管纤维化,抑制平滑肌细胞增殖,钙化和促进凋亡和改善血流速度等作用机制治疗AS。研究结果验证了脾胃虚弱升降失常是AS的本,瘀血内生,血行受阻是AS的标,“调补脾胃”的中医思想和策略为指导中医药学AS的临床应用和研究提供理论支撑。
To guide the clinical application and research on traditional Chinese medicine(TCM)in the treatment of atherosclerosis(AS),this study analyzes and summarizes classical TCM texts on spleen and stomach theory,proposing the academic viewpoint that spleen-stomach deficiency and dysfunction in ascending and descending functions constitute the core pathogenesis of AS,and that regulating and tonifying the spleen and stomach is the key to its prevention and treatment.Taking chest impediment as an example,prescriptions recorded for its treatment in TCM classics were retrieved from the Traditional Chinese Medicine Integrated Database(TCMID),then structurally processed and statistically analyzed.The study also reviews basic research progress on the treatment of AS through regulating and tonifying spleen and stomach in TCM.Results show that most prescriptions used for treating AS contain drugs that regulate and tonify the spleen and stomach,with effects such as warming the middle to dispel cold,and tonifying qi to strengthen the spleen.Chinese medicines treat AS through multiple mechanisms,including inhibiting endothelial cells(ECs)injury and protecting ECs,regulating vasodilation and vasoconstriction,suppressing vascular fibrosis,inhibiting vascular smooth muscle cell(VSMC)proliferation,calcification,and promoting apoptosis,as well as improving blood flow velocity.These findings confirm that spleen-stomach deficiency and disordered ascending and descending functions are the root of AS,while blood stasis and impaired circulation are its manifestations.The TCM strategy of“regulating and tonifying the spleen and stomach”provides a theoretical foundation for the clinical application and research of TCM in managing AS.
