摘要："培土制水法"是基于中医五行制化理论下有关脾肾疾病的具体治法体现,肾性水肿是肾脏疾病多发的临床症状,其具有病机复杂、疗效差、复发率高等特点,就其治疗而言,目前西医主要以激素及免疫抑制等为主,但其副作用较大。基于此,本文从中医基础理论"整体观念、辨证论治"框架下,从肾性水肿的理论溯源、中医病因病机特点及治疗体会等方面出发,探索"培土制水法"在肾性水肿中的临证应用,导师自拟芪苓健脾汤是该法指导下对肾性水肿治疗的新方药,笔者在临证应用中深刻体会到此方对肾性水肿脾虚水停证疗效显著,故从"培土制水法"指导下的肾性水肿的理论研究与临证心得展开论述,以期进一步指导临床。
The method of "cultivating soil to make water" is based on the specific treatment of spleen and kidney diseases under the theory of the five elements of traditional Chinese medicine. Renal edema is a frequent clinical symptom of kidney diseases, which has the characteristics of complex pathogenesis, poor curative effect and high recurrence rate. As far as its treatment is concerned, western medicine mainly focuses on hormones and immunosuppression, but its side effects are serious. Based on this, under the framework of "overall concept, syndrome differentiation and treatment" of the basic theory of traditional Chinese medicine, and from the theoretical origin, etiology, pathogenesis and treatment experience of renal edema, this paper explored the clinical application of "cultivating soil to make water" in the treatment of renal edema. Qiling Jianpi Decoction is a new prescription for the treatment of renal edema under the guidance of this method. In the clinical application, the author deeply realized that this prescription is effective in the treatment of renal edema and spleen deficiency water stagnation syndrome, and so it is discussed from the theoretical research and clinical experience of renal edema under the guidance of "cultivating soil to make water", in order to further guide the clinic.
