摘要:代谢相关脂肪性肝病作为全球高发的慢性代谢性疾病,其发病机制复杂且缺乏特效药物。本文基于中医“肝脾相关”与“伏毒损络”理论,探讨“肝脾失调-伏毒内生-络脉损伤”的病机演变与代谢相关脂肪性肝病“多重打击”学说对应关系:肝脾失调作为代谢紊乱的始动病机,对应“初次打击”的胰岛素抵抗与脂代谢紊乱;湿、痰、瘀、浊久蕴化毒形成“伏毒内生”,对应“二次打击”的氧化应激和炎症级联反应;络脉损伤致微循环障碍与结构破坏,对应“终末打击”的肝纤维化。该理论为代谢相关脂肪性肝病分期辨证提供理论依据,为其防治提供新思路。
Metabolic associated fatty liver disease as a globally prevalent chronic metabolic disease,has a complex pathogenesis,and lacks specific drugs.This article is based on theories of“liver and spleen correlation”and“latent pathogenic factors damaging collaterals”in traditional Chinese medicine,explores correspondence between pathological evolution of“liver and spleen disharmony-latent pathogenic factors endogeny-collateral damage”and“multiple-strike”theory of metabolic associated fatty liver disease:liver and spleen disharmony as initiating pathogenesis of metabolic disorders,corresponds to insulin resistance and lipid metabolism disorders in“the first strike”;dampness,phlegm,blood stasis,and turbidity accumulate for a long time to form a“latent pathogenic factors endogeny”,corresponding to oxidative stress,and inflammatory cascade reaction of“the second strike”;collateral damage leads to microcirculatory disorders and structural damage,corresponding to liver fibrosis caused by“terminal strike”.This theory provides a theoretical basis for staged differentiation of syndromes in metabolic related fatty liver disease,and offers new ideas for its prevention and treatment.
