摘要:目的:调查分析兰州市汉族、回族、东乡族的体检人群中血清尿酸(uric acid,UA)水平差异,明确饮食、体质等影响因素间的相关性,为高尿酸水平人群的饮食干预提供科学依据.方法:选取2015年1月~2018年1月于西北民族大学医院、甘肃中医药大学附属医院参加健康体检的回族985例、东乡族626例和汉族1654例(两家医院的第一次体检人群),均用生化分析仪测定UA水平,按年龄段分为青年组、中年组和老年组,比较分析并调查体检者基本信息、饮食生活、身体测量等资料.结果:青、中、老年组男性平均UA水平均高于相同年龄段女性平均UA水平(P<0.05),东乡族、回族男性UA水平均高于汉族男性UA水平(P<0.05);三组男性平均UA水平比较:东乡族>回族>汉族(P<0.05),HUA的检出率:东乡族>回族>汉族(P<0.05);青年组、中年组女性平均UA水平比较:东乡族>回族>汉族(P<0.05);老年组女性UA水平比较:东乡族和回族无差异(P>0.05),均高于汉族女性(P<0.05),HUA的检出率:东乡族>回族>汉族(P<0.05).老年组女性HUA检出率高于男性(P<0.05).对肉类(牛、羊肉及内脏)、油炸食物、肉汤(牛、羊肉)饮食、血压、民族、体质指数进行logistic回归分析显示:肉类(牛、羊肉及内脏)(OR=1.872,95%CI:1.031~2.820)、油炸食物(OR=1.079,95%CI:1.089~1.774)、肉汤(牛、羊肉)(OR=2.076,95%CI:1.532~2.921)、民族(OR=1.440,95%CI:1.232~1.774)、体质指数(OR=2.428,95%CI:1.991~3.029)、血压(OR=1.400,95%CI:1.106~1.697).结论:兰州市汉族、回族、东乡族体检人群中血尿酸水平在民族、性别和年龄中存在差异;肉类、油炸食物、肉汤、血压、民族、体质指数与高血尿酸水平具有相关性.
ObjectiveTo investigate the difference of serum uric acid(UA) levels in the physical examination population of the different nationalities, genders and ages in lanzhou city, and to determine the correlation between diet and physical conditions. Methods 985 health cases of Hui nationality, 626 cases of Dongxiang nationality and 1 654 cases of Han nationality were selected from January 2015 to January 2018 in the second people’s hospital of Gansu province(the affiliated hospital of Northwest Minzu University),and the affiliated hospital of Gansu University of Traditional Chinese Medicine, who were allocated into youth group, middle-aged group and elderly group.UA levels were determined with a biochemical analyzer.Meanwhile, the physical examination of basic information, diet, lifestyle, physical measurement and other data were collected in all population.Results The average UA levels of men of all nationality groups were higher than that of women of the same age group(P<0.05).UA levels of both minority groups in men population were higher than that of Han nationality(P<0.05).Comparison of average UA levels among men of various nationality groups in the youth group, middle-aged group and elderly group showed: Dongxiang nationality>hui nationality>han nationality(P<0.05),HUA detection rate: dongxiang nationality>Hui nationality>Han nationality(P<0.05).Comparison of average UA levels among females of the different enationality groups in the youth and middle age groups presented: Dongxiang nationality>Hui nationality>Han nationality(P<0.05).Comparison of UA levels among females in the elderly group indicated that there was no difference between Dongxiang nationality and Hui nationality(P>0.05),both of which were higher than those of Han nationality(P<0.05).Detection rate of HUA was Dongxiang nationality>Hui nationality>Han nationality(P<0.05).The detection rate of the women of all nationality groups in elderly group was higher than that of men(P<0.05).Logistic regression analysis of meat(beef, mutton and internal organs),fried food, meat soup(beef and mutton) diet, blood pressure, ethnicity and body mass index showed that meat(beef, mutton and viscera)(OR=1.872,95% CI:1.031~2.820),fried food(OR=1.079,95% CI:1.089~1.774),broth(cattle, sheep)(OR=2.076,95% CI:1.532~2.921),the national(OR=1.440,95% CI: 1.232~1.774),body mass index(OR=2.428,95% CI:1.991~3.029),lood pressure(OR=1.400,95% CI:1.106~1.697).Conclusion There are ethnic, gender and age differences in the blood uric acid levels of nationalities and han nationality in lanzhou.Meat, fried food, broth, blood pressure, ethnicity and body mass index were correlated with high blood uric acid level.
