摘要：糖尿病肾病(DN)是糖尿病(DM)最严重的慢性微血管并发症之一,中老年患者发病率高。“气之盛衰”是《黄帝内经》对于人一生生理性气血盛衰变化机制的高度概括,基于中老年人五脏之气衰减理论,认为DN病程变化会影响到五脏之气的生成与布散,且五脏之气的衰减又会加快DN病程进展,两者相互影响,成为中老年DN难治的主要原因。“治未病”理论是最具中医特色的治疗理念,其理论蕴含丰富,贯穿疾病始终。在治疗DN现证的基础上,不同年龄段巩固相应未受损的脏气,做到未病先防,或补充已衰减的脏气,做到既病防变,防治结合,从而治疗中老年DN。
Diabetic nephropathy(DN)is one of the most serious chronic microvascular complications of diabetes mellitus(DM),with a high incidence in middle-aged and elderly patients.“The rise and fall of qi”is a highly generalized description of the physiological rise and fall of qi and blood in the Huangdi Neijing.Based on the five internal organs in the elderly in the breath of attenuation theory,it is believed that the change of DN course will affect the generation and distribution of qi of the five-zang organs,and the qi attenuation of the five-zang organs will accelerate the progress of DN course.The two influence each other and become the main reason why the middle and old people are difficult to treat DN.The theory of“preventive treatment of disease”is the most characteristic of traditional Chinese medicine.It contains rich theory and runs through the whole disease.On the basis of treating the present syndrome of DN,the corresponding undamaged viscera qi in different age groups should be consolidated to prevent the disease before it occurs,or the attenuated viscera qi should be supplemented to prevent the disease from changing and combine prevention and treatment,so as to treat the middle-aged and elderly DN.
