摘要：腰椎间盘突出症(lumbar disc herniation,LDH)是骨科最常见的病变之一。随着人们生活方式的改变,日趋年轻化,其发病率也呈逐渐上升的趋势,不仅影响着日常生活和工作,而且也增加了社会负担。西医治疗以手术和保守治疗为主,但具有一定的局限性和风险性,而中医在治疗LDH方面已逐渐发挥出了特有的优势,其中“三步三位九法”手法是临床中常用的有效方法之一,其机理主要是结合人体脊柱力学平衡的特性。文章旨将结合人体脊柱力学平衡特点,概括以“三步三位九法”手法维持LDH动、静态平衡系统之间的力学源性平衡,恢复患者筋骨失衡,为手法干预LDH在临床上提供一定的理论依据。
Lumbar disc herniation(LDH)is one of the most common diseases in orthopedics.With the change of people′s lifestyle,they are getting younger and younger,and their incidence rate is also on the rise,which not only affects their daily life and work,but also increases the social burden.Western medicine mainly focuses on surgery and conservative treatment,but it has certain limitations and risks.Traditional Chinese medicine has gradually demonstrated its unique advantages in treating LDH,among which the“three-step,three-position and nine-method”manipulation is one of the commonly used effective methods in clinical practice,and its mechanism is mainly based on the characteristics of human spinal mechanics balance.This article aimed to summarize the mechanical balance between the dynamic and static balance systems of LDH using the“three-step,three-position and nine-method”manipulation based on the characteristics of human spinal mechanical balance,to restore the patients′musculoskeletal imbalance and provide a theoretical basis for manual intervention in LDH in clinical practice.
