摘要:《黄帝内经》对水肿病机的探析,可归结为阴阳不和,脏腑失调,但若再进一步分析总结,可知水肿的发生最本质的机理在于气,如阴阳不合之"五脏阳以竭"者,或因阳气虚衰不能气化,或因阳气阻遏不能布散;其本在肾者,不过肾虚不能气化;其标在肺者,无非肺气失于宣降;其制在脾者,皆因脾气失于运化;关于三焦及肝者,气机阻滞,水道不畅;关于瘀血者,瘀阻气滞而气不行水。故而水肿之疗贵在温阳利水,但绝非仅此一法,还可从肺发汗,如仲景之大青龙汤,从脾健脾消肿,如严用和之实脾饮,从瘀加活血化瘀之品等其他之法,而具体可责之于肾、肺、脾、肝、三焦等。
The analysis of the pathogenesis of edema in Huangdi's Canon of Medicine can be attributed to yin-yang disharmony and internal disorders of viscera. However,for further analysis and summary,it can be seen that the most essential mechanism of edema is qi,such as viscera yang exhaustion due to yin-yang incompatibility,or dysfunctional activity of qi due to deficient yang qi,or obstruction of yang qi leading to distribution of qi. If disease cause lies in the kidney,deficient kidney leads to failure in functional activity of qi. If disease cause lies in the lung,lung qi fails to dispel and descend. If disease cause lies in the spleen,spleen qi fails to transport and transform. If disease cause lies in sanjiao and the liver,qi activity is blocked and fluid passage is unsmooth. If disease cause lies in blood stasis,qi obstruction occurs. So treatment of edema focuses on warming yang and promoting urination,besides,sweating based on lung condition can be used,for example,ZHANG Zhong-jing's Major Green Dragon Decoction. Activating the spleen to relieve edema can be used,for example,YAN Yong-he's Spleen-Strengthening Decoction. Activating blood circulation and removing stasis can also be used,while the exact related fu or zang organs lie in the kidney,lung,spleen,liver,sanjiao,etc.
