摘要：骨质疏松症(osteoporosis,OP)是以骨稳态失衡、骨结构破坏和骨折风险增加为主要特征一种全身性骨骼疾病。最新研究表明脂质与骨代谢平衡之间关系密切,脂质稳态可能对于OP的发生发展及预后有重要意义。脂质稳态是指血液或其他组织中脂质及其代谢物的定性与定量变化。中医“五津之榖液和合而为膏者,内渗入于骨空,补益脑髓,而下流于阴股”认为精液和合为膏,养于骨骼,上至脑窍,涵养脑髓,下达肾窍,滋润阴精,以发挥其充脑髓,养肾窍,化精血之功效。“津液为膏,内渗于骨”理论是中医对于脂质代谢与骨代谢关系的阐释,该文在“津液为膏,内渗于骨”理论指导下,论述脂质稳态与OP相关性的研究进展,旨在丰富“津液为膏,内渗于骨”理论的现代科学内涵,为OP的防治带来新的机会和途径。
Osteoporosis(OP)is a systemic bone disorder characterized by an imbalance in bone homeostasis,the destruction of bone structure,and an increased risk of fractures.Recent research has demonstrated a significant correlation between lipid metabolism and bone health,suggesting that lipid homeostasis may play a crucial role in both the pathogenesis and prognosis of OP.Lipid homeostasis encompasses qualitative and quantitative alterations in lipids and their metabolites present in blood or other tissues.In Traditional Chinese Medicine(TCM),it is believed that the essence derived from grains and fluids nourishes the five organs,permeating into the medullary cavities to nourish the brain while descending to enrich Yin and blood.This perspective holds that seminal fluid amalgamates into an ointment-like substance that nourishes both brain function and kidney vitality while moistening Yin essence,thus fulfilling its roles in enhancing cerebral function,supporting renal health,and purifying essence as well as blood.The concept of body fluids acting as an ointment for endoosmosis to the bone serves as an interpretative framework linking lipid metabolism with bone metabolism within TCM.Guided by this principle,this paper explores recent advancements regarding to the interplay between lipid homeostasis and OP with the aim of enriching contemporary scientific understanding of body fluids as ointments for endoosmosis to the bone,thereby presenting new avenues for prevention and treatment strategies against OP.
