摘要:急性胰腺炎(AP)是消化科常见急症之一,由多种原因引起,发生机制复杂,若不及时治疗可导致呼吸衰竭甚至多脏腑功能衰竭,病情严重可致患者死亡。现阶段临床治疗AP仍采用传统西医常规治疗,但其效果有待加强。有大量文献报道及实验数据显示,大黄牡丹汤医治AP患者取得良好的疗效。通过查阅中外有关文献及研究成果,从抑制胰腺的分泌、调控炎性细胞因子、保护肠黏膜、促进细胞凋亡、促进胰腺微循环、氧化应激反应、降低胰腺腺泡内钙超载等方面,对大黄牡丹汤治疗AP的作用机制进行综述,以期为AP临床治疗的发展提供参考。
Acute pancreatitis(AP)is one of the common emergencies in the digestive department.It is caused by many reasons and its pathogenesis is complex.If not treated in time,it can lead to respiratory failure and even multiple viscera failure.If the condition is serious,it can lead to death.At present,the clinical treatment of AP is still conventional treatment of traditional western medicine,but its effect needs to be strengthened.A large number of literature reports and experimental data show that Dahuang Mudan Decoction(大黄牡丹汤)has achieved good effect in treating AP patients.By consulting the relevant literature and research results at home and abroad,the mechanism of Dahuang Mudan Decoction in the treatment of AP was reviewed from the aspects of inhibiting the secretion of pancreas,regulating inflammatory cytokines,protecting intestinal mucosa,promoting cell apoptosis,promoting pancreatic microcirculation,oxidative stress reaction and reducing calcium overload in pancreatic acini,with a view to providing reference for the development of clinical treatment of AP.
