摘要：目的观察郑氏"金钩钓鱼法"治疗顽固性面瘫的临床疗效。方法将90例顽固性面瘫患者运用随机数字表法分为金钩钓鱼组和平补平泻组,每组45例。金钩钓鱼组取患侧头维、攒竹、太阳、阳白、四白、颧髎、颊车、阿是穴、牵正、迎香、风池、翳风,双侧合谷、足三里、太冲穴,毫针刺入后在有筋结或条索状的穴位及阿是穴操作"金钩钓鱼法",每次选穴3～4个,余穴采用平补平泻法;平补平泻组取穴同金钩钓鱼组,针刺用平补平泻法。治疗前后通过Sunnybrook面神经评分、H-B面神经功能评价及治疗后总有效率评定疗效。结果金钩钓鱼组总有效率为93.3%,平补平泻组为64.4%,金钩钓鱼组疗效更优(P<0.01)。两组治疗后Sunnybrook面神经评分、H-B面神经功能评价均有不同程度改善(P<0.05,P<0.01),金钩钓鱼组改善优于平补平泻组(P<0.05,P<0.01)。结论 "金钩钓鱼法"能明显改善顽固性面瘫患者的症状和体征,具有较好的临床效果。
Objective To investigate the clinical efficacy of Zhen’s“gold hook fishing” therapy for intractable facial paralysis. Method Ninety patients with intractable facial paralysis were allocated, using a random number table, to gold hook fishing and even reinforcement and reduction groups, 45 cases each. In the gold hook fishing group, points Touwei （ST8）, Zanzhu （BL2）, Taiyang （Ex-HN5）, Yangbai （GB4）, Sibai （ST2）, Quanliao （SI18）, Jiache （ST6）, Ashi, Qianzheng （Ex-HN16）, Yingxiang （LI20）, Fengchi （GB20） and Yifeng （TE17） on the affected side and points Hegu （LI4）, Zusanli （ST36） and Taichong （LR3） on both sides were selected;“gold hook fishing method” was performed at tendon knot- or stripe-shaped acupoints after insertion of filiform needles; three to four acupoints were selected each time; the even reinforcement and reduction method was performed at the other acupoints. In the even reinforcement and reduction group, the same acupoints as in the gold hook fishing group were selected and the even reinforcement and reduction method was used for acupuncture. The therapeutic effects were evaluated using the Sunnybrook Facial Grading System, the H-B facial nerve grading system and the total clinical efficacy rate. Result The total efficacy rate was 93.3% in the gold hook fishing group and 64.4% in the even reinforcement and reduction group. The therapeutic effect was better in the gold hook fishing group （P〈0.01）. The Sunnybrook Facial Grading System score increased andthe H-B facial grading system-based facial nerve function improved in different degrees in the two groups after treatment. They increased and improved more, respectively, in the gold hook fishing group （P〈0.05）. Conclusion “Gold hook fishing” therapy can significantly relieve the symptoms and signs in patients with intractable facial paralysis. It has a better clinical effect.
