摘要:目的观察热补针法对类风湿关节炎(RA)寒证模型兔膝关节滑膜组织沉默信息调节因子3(SIRT3)、超氧化物歧化酶2(SOD2)和过氧化氢酶(CAT)表达的影响,探讨其干预RA的机制。方法将30只新西兰兔随机分为正常组、模型组、平补平泻组、捻转补法组和热补针法组,每组6只。正常组、模型组不予针刺,其余各组选取双侧“足三里”分别施以平补平泻法、捻转补法、热补针法,行针1 min,留针30 min,1次/d,连续7 d。观察兔一般情况,测定痛阈;彩超观察膝关节滑膜变化、积液及异常血流信号;HE染色观察膝关节滑膜组织病理改变;RT-PCR检测膝关节滑膜组织SIRT3、SOD2、CAT mRNA表达。结果与正常组比较,模型组兔精神萎靡,体毛杂乱黯淡,膝关节肿胀增大,活动量减少,痛阈明显降低(P<0.05);滑膜增厚明显,有大量积液,可见关节囊肿胀;滑膜被覆细胞排列紊乱,滑膜基质重度增生,炎性细胞浸润明显;膝关节滑膜组织SIRT3、SOD2、CAT mRNA表达明显升高(P<0.05)。与模型组比较,各针刺组兔精神状态有所恢复,体毛杂乱黯淡有所改善,关节肿胀减轻,痛阈明显升高(P<0.05);滑膜厚度、积液、滑膜被覆细胞形态、滑膜基质状态,炎性细胞浸润均明显改善;膝关节滑膜组织SIRT3、SOD2、CAT mRNA表达明显降低(P<0.05);以热补针法组作用更明显(P<0.05)。结论热补针法通过相对特异性下调RA兔膝关节滑膜组织SIRT3、SOD2、CAT表达,增强其抗氧化能力,从而发挥治疗RA的作用。
Objective To observe the effects of heat reinforcing needling on the expressions of SIRT3,SOD2 and CAT in synovium of knee joint of rabbits with rheumatoid arthritis(RA)cold syndrome;To explore the mechanism of intervening RA.Methods Totally 30 New Zealand rabbits were randomly divided into normal group,model group,reinforcing-reducing needling group,twirling-reinforcing needling group,and heat-reinforcing needling group,with 6 cases in each group.The normal group and the model group were not given acupuncture treatment,while the other groups were performed with the method of reinforcing-reducing needling,twirling-reinforcing needling,and heat-reinforcing needling on both sides at“Zusanli”respectively.The acupuncture was performed for 1 min,and the needle was retained for 30 min,once a day,for 7 days.The general condition of rabbits was observed,the pain threshold was measured;the synovium changes,hydrops and abnormal blood flow signals of knee joint were observed by color Doppler ultrasound;the histopathological changes of the synovium of knee joint were observed by HE staining;the expressions of SIRT3,SOD2,CAT mRNA in synovial tissue of knee joint was detected by RT-PCR.Results Compared with the normal group,the rabbits in the model group were mentally sluggish,the body hair was messy and dull,the knee joints swelled and increased,and with fewer activities,the pain threshold was significantly decreased(P<0.05);the synovium was obviously thickened,there was a large amount of hydrops,the joint capsule was swollen;the synovial covering cells arrangement was disordered,the synovial matrix was heavily proliferated,and the inflammatory cells infiltration was obvious;the expressions of SIRT3,SOD2,CAT mRNA in synovial tissue of knee joint significantly increased(P<0.05).Compared with the model group,the mental state of the rabbits in each acupuncture group recovered,the disordered and dim body hair was improved,the swelling of the joints was reduced,the pain threshold significantly increased(P<0.05);the thickness of synovium,hydrops,morphology of synovial covering cells,state of synovial matrix,and the infiltration of inflammatory cells were significantly improved;the expressions of SIRT3,SOD2,CAT mRNA in synovial tissue of knee joint significantly decreased(P<0.05).The effect was more obvious in heat-reinforcing needling group(P<0.05).Conclusion Heat-reinforcing needling can enhance the antioxidant capacity and play a role in the treatment of RAby relatively specifically down-regulating the expressions of SIRT3,SOD2,CAT in knee synovial tissue of RArabbit.
