摘要:肝纤维化是多种病因引起肝细胞受损,导致慢性肝脏病进展至肝硬化或肝癌不可避免的病理过程,严重影响患者生命健康及生活质量。研究表明,肝星状细胞的激活以及细胞外基质的过量积累是导致肝纤维化的主要原因,而细胞因子、细胞传导信号通路以及免疫自噬对肝纤维化过程有重要影响。近年来中药在防治肝脏疾病中广泛应用并有良好的疗效,研究发现,中药以其多途径、多靶点、多层次的特点可对TGF-β/Smad通路、P13K/Akt通路、MAPK通路、NF-κB通路及Wnt等信号通路产生影响进而抑制肝星状细胞活化,减少细胞外基质增生,降低肝脏内炎症因子水平等,达到防治肝纤维化的作用。就中药干预肝纤维化相关信号通路研究现状进行归纳总结,以期为肝纤维化的发病研究及中药治疗策略提供新的思路和方向。
Hepatic fibrosis is an inevitable pathological process of chronic hepatic disease progression to cirrhosis or hepatic cancer caused by multiple causes of hepatocyte damage,which seriously affects patients'life health and quality of life.Studies have shown that hepatic stellate cells activation and excessive accumulation of extracellular matrix are the main causes of hepatic fibrosis.Cytokines,cell signaling pathways and immune autophagy have important effects on the process of hepatic fibrosis.In recent years,traditional Chinese medicine has been widely used in the prevention and treatment of hepatic diseases and has good curative effect.Studies have found that traditional Chinese medicine,with its multi-pathway,multi-target and multi-level characteristics,can affect TGF-β/Smad pathway,P13K/Akt pathway,MAPK pathway,NF-κB pathway and Wnt signaling pathway to inhibit the activation of hepatic stellate cells,reduce the proliferation of extracellular matrix,reduce the levels of inflammatory factors in the liver and achieve the role of prevention and treatment of hepatic fibrosis.This article summarized the current research status of the signal pathways related to the intervention of traditional Chinese medicine in hepatic fibrosis,in order to provide new ideas and directions for the pathogenesis research of hepatic fibrosis and the strategy of traditional Chinese medicine diagnosis and treatments.
