摘要：中医将乳腺癌巩固期辨证分为肝郁脾虚证、脾肾亏虚证、冲任失调证、气血亏虚证、气阴两虚证、厥阴证、肾虚血瘀证等证型,分别给予对应方药治疗。乳腺癌巩固期的中医外治法有:针灸、艾灸、穴位贴敷、中药足浴、情志疗法、导引治疗等。乳腺癌巩固期是乳腺癌治疗极为重要的阶段,良好的治疗效果需要患者及医生很好地配合,才能减轻病痛,巩固疗效,防止复发转移。长期治疗会出现类更年期症状,加大患者的精神压力及临床不适症状,同时长期内分泌治疗极容易产生耐药性,使后期疗效减弱。中医药治疗基于“整体观念”可以全面调控患者的状态,但中医辨证论治药物作用机制不明,缺少基础研究相关证据,且相关临床研究所涉样本量较小,易产生偏倚;其他中医疗法尚处于探索阶段,体系还不够完备;加之临床医生受技术条件的限制。因此,基于基础研究和多中心大样本的临床研究建立一个成熟的乳腺癌巩固期中医药治疗体系迫在眉睫。
TCM classifies the consolidation-phase breast cancer into such syndrome differentiation types as liver stagnation and spleen deficiency,spleen-kidney deficiency,TV-CV disharmony,qi-blood deficiency qi-yin deficiency,reverting yin syndrome,and kidney deficiency with blood stasis.Each syndrome is treated with corresponding drugs and formulas.External TCM therapies for the consolidation-phase breast cancer include acupuncture,moxibustion,acupoint application,Chinese medicinals for foot baths,emotional therapy,and conducting exercise,etc.The consolidation phase is a critical stage in breast cancer treatment,requiring close collaboration between patients and physicians to alleviate symptoms,consolidate efficacy,and prevent recurrence or metastasis.Long-term treatment may induce menopausal-like symptoms,exacerbating emotional stress and clinical discomfort,while prolonged endocrine therapy often leads to drug resistance,diminishing later-stage efficacy.TCM,guided by"the concept of holism"can comprehensively regulate patients′conditions.However,the mechanisms of action of TCM syndrome-specific treatments remain unclear,with insufficient foundational research evidence.Besides,existing clinical studies often involve small sample sizes,increasing bias risks.Other TCM therapies are still exploratory,lacking systematic frameworks,and clinical practice is constrained by technical limitations.Therefore,it is urgently needed to establish a mature TCM treatment system for consolidation-phase breast cancer,supported by foundational research and multicenter large-sample clinical trials.
