摘要:目的:探讨半夏泻心汤对多囊卵巢综合征(Polycystic ovary syndrome, PCOS)胰岛素抵抗(Insulin resistance, IR)模型大鼠肠道菌群的影响。方法:采用来曲唑连续灌胃制备多囊卵巢综合征胰岛素抵抗(PCOS IR)模型,随机分成模型对照组、二甲双胍0.135 g/kg组、半夏泻心汤5.31 g/kg组以及半夏泻心汤+二甲双胍(5.31+0.135) g/kg组,另设正常对照组,连续灌胃28 d后称大鼠体质量、卵巢,计算子宫指数,观察卵巢组织病理学,检测血清性激素水平、胰岛素指数(HOMA-IR);用16s rDNA扩增子焦磷酸测序检测肠道菌群变化。结果:与正常对照组比较,模型对照组体质量、子宫指数明显升高,大鼠动情期紊乱,血清睾酮(T)含量、黄体生成素(LH)/促卵泡生成素(FSH)比值明显升高,HOMH-IR指数明显升高,血糖升高(P<0.05),大鼠肠道Chao、ACE指数明显降低,拟杆菌门/后壁菌门比值明显升高,普氏菌属丰度明显升高,苏黎世杆菌属、乳酸杆菌属、阿克曼氏菌属、双歧杆菌属丰度明显降低(P<0.05或P<0.01)。半夏泻心汤明显降低PCOS模型大鼠体质量和子宫指数,降低血清LH、T含量、胰岛素指数(HOMA-IR)(P<0.05);增加肠道菌群数量、多样性以及菌群均匀度,尤其是增加厚壁菌门、变形菌门、放线菌门以及拟杆菌属、颤螺菌属、粪球菌属、罗斯氏菌属、乳酸菌属、萨特氏菌属、双歧杆菌和阿克曼菌属相对丰度(P<0.05);降低拟杆菌门和蓝藻菌门、普氏菌属和厌氧菌属相对丰度(P<0.05)。结论:半夏泻心汤对PCOS IR的调控作用可能与调整肠道菌群,提高有益菌相对丰度、降低致病菌相对丰度,降低血清雄激素和胰岛素水平有关。
Objective:To investigate the effect of Banxia Xiexin decoction(半夏泻心汤)on intestinal flora in polycystic ovary syndrome(PCOS)rats with insulin resistance(IR).Methods:The rat model of POCS with IR(PCOS IR)was established by continuous administration of letrozole by gavage.The rats were randomly divided into model group,metformin group(0.135 g/kg),Banxia Xiexin decoction group(5.31 g/kg)and Banxia Xiexin decoction+metformin group(5.31+0.135 g/kg),and normal control group.The body weight,ovarian and uterine indexes,ovarian histopathology,serum sex hormone levels,homeostasis model assessment of insulin resistance(HOMA-IR),and intestinal flora detected by 16s rDNA amplicome pyrosequencing were analyzed after 28 days of continuous gavage.Results:Banxia Xiexin decoction significantly reduced the body weight, uterine index, serum luteinizing hormone (LH) and testosterone (T)levels, and HOMA-IR level (P<0.05), and increased the number, diversity and evenness of intestinal florain PCOS model rats. In particular, the relative abundance of Firmicutes, Proteobacteria, Actinobacteria,Bacteroides, Oscillospira, Coprococcus, Roseburia, Lactobacillus, Sutterella, Bifidobacterium andAkkermansia muciniphila was increased(P<0.05), while the relative abundance of Bacteroidetes andCyanobacteria, Prevotella and Anaeroplasma was reduced by Banxia Xiexin decoction(P<0.05).Conclusion: The effect of Banxia Xiexin decoction on PCOS IR may be related to the regulation ofintestinal flora, increase of the relative abundance of beneficial bacteria, decrease of the relative abundanceof pathogenic bacteria, and decrease of the serum androgen and insulin levels.
