摘要:目的比较健康女性阴道不同部位pH值、菌群多样性、密集度、优势菌及局部机体反应状态的差异,阴道分泌物取材对临床诊断的指导意义。方法在普查人群中,通过阴道分泌物涂片镜检排除阴道感染的健康妇女30例,分别在阴道侧壁上1/3处、阴道后穹窿、阴道下1/3处三处取材后进行微生态分析。结果不同部位取材后检验差异不大,但是在取材时存在取材量的不同,拭子饱和度后穹窿较多,阴道下1/3次之,尤以阴道上1/3处拭子饱和度最小。菌群密集度和多样性分析三者之间有差异,乳酸杆菌多在上1/3处居多,阴道后穹窿和阴道下1/3处杂菌相对较多。结论诊断性取样应以阴道上1/3侧壁为准,能真正反映阴道菌群的状态及致病菌。同时在分泌物涂片时应以薄均匀为准,以免影响染色的效果。
Objective To compare the difference of vaginal PH values, bacterial diversity, bacterial intensity, dominant bacteria and reactive state of local body in different vaginal positions of healthy women, as well as the significance of vaginal secretions sampling in clinical diagnosis. Methods In the census population, vaginal secretions were sampled from 30 healthy women, excluding vaginal infections through vaginal secretions smear microscopy, at upper 1/3 of the vaginal wall, the posterior vaginal fomix, lower 1/3 of the vagina for microbial analysis. Results The test results of vaginal secretions sample had little difference at different positions. However, sampling amount showed large difference at different positions. The saturation level of swabs showed highest at posterior vaginal fomix, fallowed by upper 1/3 of the vaginal wall and lower 1/3 of the vagina. The flora intensity and flora diversity differed among the three positions, laetobaeilli was more at upper 1/3 of the vaginal wall, while sundry bacteria was more at posterior vaginal fornix and lower 1/3 of the vagina. Conclusion Diagnostic sampling should be subject to 1/3 side wall of vagina, because it can truly reflect the state of the vaginal flora and pathogens. Thin and uniform pap smear should prevail to avoid influencing the dyeing effect.
