摘要：目的观察中药离子导入联合低频脉冲治疗气阴两虚型糖尿病患者干眼的临床疗效,为中医药治疗糖尿病患者干眼提供治疗依据。方法收集2019年1月—2019年6月符合纳入标准的气阴两虚型糖尿病患者干眼80例(160只眼),随机分为治疗组和对照组各40例(80只眼)。对照组给予玻璃酸钠滴眼液点眼,治疗组在对照组基础上给予中药离子导入联合低频脉冲治疗。观察2组主观症状、泪河高度(TMH)、泪膜破裂时间(BUT)、泪液分泌试验(SIT)、角膜荧光素染色(FL)指标,并对治疗前后进行对比、分析。结果(1)主观症状:2组治疗后组间比较,干涩感(Z=-2.376,P=0.002)、异物感(Z=-2.863,P=0.000),差异均有统计学意义;疲劳感(Z=-1.536,P=0.054),差异无统计学意义。治疗前后组内比较,治疗组患者干涩感(Z=-5.598,P=0.002)、异物感(Z=-5.709,P=0.000)、疲劳感(Z=-5.463,P=0.004);对照组患者干涩感(Z=-5.563,P=0.002)、异物感(Z=-5.858,P=0.000)、疲劳感(Z=-5.482,P=0.004),差异均有统计学意义。(2)干眼指标:2组治疗后组间比较,TMH(Z=-2.873,P=0.042)、SIT(Z=-3.524,P=0.004)、BUT(Z=-3.147,P=0.016)、FL(Z=-2.964,P=0.024),差异均有统计学意义。治疗前后组内比较,治疗组患者TMH(Z=-7.373,P=0.033)、SIT(Z=-7.682,P=0.020)、BUT(Z=-7.756,P=0.011)、FL(Z=-7.360,P=0.033);对照组患者TMH(Z=-6.671,P=0.034)、SIT(Z=-6.655,P=0.037)、BUT(Z=-7.723,P=0.018)、FL(Z=-7.501,P=0.030),差异均有统计学意义。(3)临床疗效:治疗组显效23例,有效14例,无效3例;对照组显效12例,有效20例,无效8例。2组比较,Z=-2.578,P=0.010,等级差异具有统计学意义。结论中药离子导入联合低频脉冲在治疗2型糖尿病患者干眼中具有显著的临床疗效。
OBJECTIVE To observe the clinical efficacy of Chinese medicine iontophoresis combined with low-frequency pulse therapy in the treatment of diabetes with dry eye diagnosed as qi and yin deficiency syndrome,and to provide references for the treatment of dry eye of the diabetic patients with Traditional Chinese Medicine.METHODS From January 2019 to June 2019,80(160 eyes)dry eye of diabetic patients with qi and yin deficiency type who met the inclusion criteria were collected and randomly divided into treatment group and control group with 40 cases(80 eyes).The control group was given sodium hyaluronate eye drops,and the treatment group was treated by Chinese medicine iontophoresis combined with low-frequency pulse therapy on the basis of the control group.The subjective symptoms of the two groups,tear river height(TMH),tear film rupture time(BUT),Schirmer I test(SIT),corneal fluorescein staining(FL)indicators were observed,and compared and analyzed before and after treatment.RESULTS(1)Subjective symptoms:comparison between the two groups after treatment showed that the difference of dryness(Z=-2.376,P=0.002)and foreign body sensation(Z=-2.863,P=0.000)were statistically significant;the difference of fatigue(Z=-1.536,P=0.054)was not statistically significant.Comparison before and after treatment showed that dryness(Z=-5.598,P=0.002),foreign body sensation(Z=-5.709,P=0.000),and fatigue(Z=-5.463,P=0.004)in the treatment group and dryness(Z=-5.563,P=0.002),foreign body sensation(Z=-5.858,P=0.000),fatigue(Z=-5.482,P=0.004)in the control group showed statistically significant difference.(2)Dry eye index:comparison between the two groups after treatment showed that differences of TMH(Z=-2.873,P=0.042),SIT(Z=-3.524,P=0.004),BUT(Z=-3.147,P=0.016),FL(Z=-2.964,P=0.024)were statistically significant.In comparison before and after treatment showed that TMH(Z=-7.373,P=0.033),SIT(Z=-7.682,P=0.020),BUT(Z=-7.756,P=0.011),FL(Z=-7.360,P=0.033)in the treatment group and TMH(Z=-6.671,P=0.034),SIT(Z=-6.655,P=0.037),BUT(Z=-7.723,P=0.018),FL(Z=-7.501,P=0.030)in the control group showed statistically significant difference.(3)Clinical effect:in the treatment group,23 cases were markedly effective,14 cases were effective,and 3 cases were ineffective;in the control group,12 cases were markedly effective,20 cases were effective,and 8 cases were ineffective.Comparison of the two groups showed the grade difference was statistically significant(Z=-2.578,P=0.010).CONCLUSIONS Chinese medicine iontophoresis combined with low-frequency pulse therapy had a significant clinical effect in the treatment of type Ⅱ diabetes patients with dry eye.
