摘要:化疗是目前治疗癌症的重要手段之一。化疗可以延长癌症患者的生存时间、提高生存率、控制肿瘤进展等,但同时也会诱发化疗后白细胞减少症。化疗导致白细胞减少症的发生主要原因是化疗使造血细胞耗竭导致的白细胞生成减少、造血微环境损伤导致的白细胞发育障碍及化疗对外周血白细胞的消耗增加。本文从这三方面出发,对其发生机制进行了详细论述,造血细胞耗竭主要从PI3K/Akt/mTOR/FoxO信号通路、TGF-β/Smad信号通路、p38MAPK通路、JAK2/STAT5信号通路论述;造血微环境损伤主要从Notch信号通路、ERK信号通路、Wnt/β-catenin信号通路、NF-κB/VCAM-1信号通路论述;化疗对外周血白细胞的损伤从Keap1/Nrf2/ARE信号通路和细胞因子的影响展开论述。以期为癌症后化疗导致的白细胞减少症的治疗提供理论参考。
Chemotherapy is one of the most important methods for the treatment of cancer.Chemotherapy can prolong the survival time of cancer patients,improve the survival rate,control the progression of tumor,and so on,but it can also induce leukopenia after chemotherapy.The occurrence of chemotherapy-induced leukopenia is mainly caused by depletion of hematopoietic cells caused by chemotherapy,leukocyte development disorder caused by damage of hematopoietic microenvironment,and increased consumption of peripheral blood leukocytes caused by chemotherapy.From these three aspects,this paper discusses the mechanism of its occurrence in detail.The depletion of hematopoietic cells is mainly discussed from PI3K/Akt/mTOR/FoxO signaling pathway,TGF-β/Smad signaling pathway and p38MAPK pathway,JAK2/STAT5 signaling pathway.The damage of hematopoietic microenvironment is mainly discussed in terms of Notch signaling pathway,ERK signaling pathway,Wnt/β-catenin signaling pathway,NF-κB/VCAM-1 signaling pathway.The mechanism of peripheral blood damage caused by chemotherapy is discussed from the influence of Keapl/Nrf2/ARE signaling pathway and cytokines,to provide theoretical reference for the treatment of leukopenia caused by chemotherapy after cancer.
