摘要:目的利用16S rDNA测序技术探讨健康与痰湿壅盛型多囊卵巢综合征(PCOS)性激素水平和肠道菌群结构差异以及半夏泻心汤的干预效应。方法选取24例痰湿壅盛型PCOS患者和同期健康女性20例,初诊留取血清和粪便标本,对PCOS患者给予半夏泻心汤治疗6个月经周期后再留取血清和粪便标本,放射免疫法检测血清性激素水平、抗苗勒管激素,采用16S rDNA扩增子测序技术对粪便标本中肠道菌群行OTUs聚类分析和物种分类分析。结果与健康女性比较,PCOS患者存在高黄体生成素(LH)和高睾酮(T)状态,肠道的拟杆菌门和变形菌门相对丰度下降,厚壁菌门和放线菌门相对丰度升高,拟杆菌属、瘤胃球菌属、双歧杆菌属、乳酸杆菌属和阿克曼菌属相对丰度下降(P<0.05),普氏菌属、毛螺菌属和柔嫩梭菌属升高(P<0.05);与PCOS组比较,半夏泻心汤中药组能明显降低PCOS患者血清LH、T和LH/FSH(卵泡刺激素)比值(P<0.05);上调拟杆菌门、变形菌门、拟杆菌属、瘤胃菌属、乳酸杆菌属、阿克曼菌属和萨特菌属相对丰度(P<0.05),下调厚壁菌门、放线菌门、普氏菌属、毛螺菌属和柔嫩梭菌属相对丰度(P<0.05)。结论半夏泻心汤有效调节PCOS性激素紊乱状态,可能的作用机制与调节肠道菌群丰度和多样性,增加有益菌,降低致病菌,恢复肠道微生态稳态有关。
Objective To explore the differences in sex hormone level and intestinal microflora structure between healthy individuals and patients with phlegm-damping-abundant polycystic ovary syndrome,and the intervention effect of Banxia Xiexin Decoction using 16S rDNA sequencing technology.Methods Twenty-four PCOS patients with excessive phlegmdampness obstruction and 20 healthy women at the same period were selected.Serum and stool samples were collected at initial diagnosis.After treatment with Banxia Xiexin Decoction for 6 menstrual cycles,serum and stool samples were collected,and the levels of serum sex hormone and anti-Müellerian hormone were detected using radioimmunoassay.OTUs cluster analysis and species classification analysis were performed with 16S rDNA amplicon sequencing.Results Compared with healthy women,PCOS patients had high luteinizing hormone and high testosterone levels,and the relative abundances of Bacteroidetes and Proteobacteria decreased,while those of Firmicutes and Actinobacteria increased.The relative abundances of Bacteroides,Ruminococcus,Bifidobacterium,Lactobacillus and Akkermansia decreased(P<0.05),while those of Prevotella,Lachnospira,Faecalibacterium increased(P<0.05).Compared with PCOS group,the ratios of serum LH,T and LH/FSH(follicle stimulating hormone)in Banxia Xiexin Decoction group significantly decreased(P<0.05).The relative abundances of Bacteroidetes,Proteobacteria,Bacteroides,Ruminococcus,Lactobacillus,Akkermania and Suttella were up-regulated(P<0.05),and those of Firmicutes,Actinobacteria,Prevotella,Lachnospira and Faecalibacterium were down-regulated(P<0.05).Conclusion Banxia Xiexin Decoction can effectively regulate the disorder of PCOS sex hormones.The possible mechanism is related to regulating the abundance and diversity of intestinal flora,increasing the relative abundance of beneficial bacteria,reducing the relative abundance of pathogenic bacteria,and restoring the intestinal microecological homeostasis.
