摘要:目的调查骨科手术后患者的院内感染病原菌种类及病原菌耐药情况。方法采集近3年骨科手术后患者感染的标本进行病原菌培养、鉴定及药敏试验。结果 387份标本共分离出145株病原菌,其中革兰阴性菌76株,占52.4%;革兰阳性菌66株,占45.5%;真菌3株,占2.1%。感染率南高到低的细菌依次是血浆凝固酶阴性葡萄球菌、大肠埃希菌、阴沟肠杆菌、铜绿假单胞菌及鲍曼不动杆菌。万古霉素对耐甲氧西林葡萄球菌抗菌作用最强,碳青霉烯类对产头孢菌素AmpC酶革兰阴性杆菌作用最强。结论骨科术后患者院内感染问题值得高度重视,应严密监测控制耐药菌株,科学合理使用抗生素;医护人员要严格执行消毒制度及无菌操作技术。
Objective To analyze the condition of pathogenic bacteria of postoperative hospitalacquired-infection in orthopedic patients and their, drug resistance. Methods Collected specimens from postoperative infectious orthopedic patients were cultured, pathogenic bacteria were identified, and drug sensitive test was performed. Results 145 strains of pathogenic bacteria were separated from 387 specimens, out of them, 76 strains were gram-negative bacteria and took 52.4%, 66 strains were gram-positive bacteria and took 45.5%, 3 strains were fungi and took 2.1%. The infection rates from high to low were plasma-coagulase negative staphylococcus, Escherichia coli, bacillus cloacae, verdigris pseudomonas and bowman's amotile bacillus. The antiseptic effect of vancomycin to methicillin-resistant staphylococcus was the strongest. Conclusion Clinical doctors should attach great importance to the issue on postoperative hospital-acquired-infection in orthopedic patients and closely survey and control the drug-resistant strains for scientific and rational use of antibiotics.
