摘要：反复呼吸道感染是指一年内发生呼吸道感染次数过于频繁的疾病。本病多见于6个月~6岁的儿童,其中1~3岁的幼儿发病率最高,学龄期前后发病次数明显减少。目前小儿反复呼吸道感染临床治疗多以抗感染、免疫调节等治疗为主,远期疗效不佳。中医药治疗本病在减少患儿复感次数,缩短病程,增强免疫功能等方面具有较好效果,且安全性和经济性佳,在减轻患儿痛苦、降低病次方面具有独特优势。甘肃中医药大学附属医院张士卿教授诊治小儿反复呼吸道感染经验丰富。张教授认为,本病发病的外因责之于外邪侵袭,内因归为肺脾气虚,外因触动内因而发。他运用“培土生金”之法,既要明辨主证,着眼于脏腑气机,以补为法,同时又要兼顾次要证候,或降或清或和,以使气机和畅,创制补脾益肺汤,临证辨治,增损斟酌,体现了张士卿教授灵活的立法组方思路,在临床应用中取得了显著疗效,值得传承与推广。文章通过案例,介绍张教授“培土生金”法治疗小儿反复呼吸道感染,以期为临床提供参考。
Recurrent respiratory infections are diseases in which respiratory infections occur too often in a year.The disease is more common in children aged 6 months-6 years,among which the incidence of children aged 1-3 years is the highest,and the number of cases before and after school age is significantly reduced.At present,the clinical treatment of recurrent respiratory tract infections in children is mainly based on anti-infection,immunomodulation and other treatments,and the long-term efficacy is not good.TCM has good effects in reducing the number of re-infections,shortening the course of the disease,enhancing immune function,etc.,and has good safety and economy,and has unique advantages in reducing the pain and disease times in children.Professor ZHANG Shiqing of the Affiliated Hospital of Gansu University of Chinese Medicine has rich experience in the diagnosis and treatment of recurrent respiratory infections in children.The professor believes that the external cause of the disease is responsible for the invasion of external evil,the internal cause is attributed to the deficiency of lung and spleen,and the external cause touches the internal cause.He uses the method of cultivating native gold,not only to distinguish the main evidence,focus on the internal organ qi machine,to supplement as the method,but also to take into account the secondary symptoms,or lower or clear or peace,so as to make the Qi movement smooth,create the Bufei Yifei decoction(补脾益肺汤),clinical evidence differentiation and treatment,increase and loss consideration,reflecting Professor ZHANG Shiqing's flexible legislative organization thinking,which has achieved significant curative effects in clinical application and is worthy of inheritance and promotion.Through a case study,this paper introduces Professor ZHANG's Peitu Shengjin method for the treatment of recurrent respiratory tract infections in children,in order to provide clinical reference.
