摘要:目的:运用系统评价方式探讨双水平气道正压通气(Bi-level Positive Airway Pressure,BiPAP)治疗慢性阻塞性肺疾病(Chronic Obstructive Pulmonary Disease,COPD)合并阻塞性睡眠呼吸暂停低通气综合征(Obstructive Sleep Apnea Hypopnea Syndrome,OSAHS)的有效性。方法:计算机检索中国知网、中国生物医学文献数据库、万方数据库、维普数据库、The Cochrane Library、Web of Science、PubMed、Embase等数据库,检索与BiPAP治疗COPD合并OSAHS相关的随机对照试验,采用RevMan 5.3对其进行Meta分析。结果:15篇文献符合纳入标准,Meta分析结果显示,BiPAP可显著改善COPD合并OSAHS患者的第1秒用力呼气容积(Forced Expiratory Volume in 1 Second,FEV_(1))占用力肺活量(Forced Vital Capacity,FVC)的百分比(FEV_(1)/FVC%)、氢离子浓度指数(pH)、动脉血氧分压(Partial Pressure of Oxygen,PaO_(2))、动脉血二氧化碳分压(Partial Pressure of Carbon Dioxide,PaCO_(2))、呼吸暂停低通气指数(Apnea Hypopnea Index,AHI)指标。结论:BiPAP可显著改善COPD合并OSAHS患者的FEV_(1)/FVC%、pH、PaO_(2)、PaCO_(2)、AHI指标,临床合理应用可显著改善患者的预后。
Objective:To explore the effectiveness of BiPAP in the treatment of COPD with OSAHS by a systematic evaluation.Methods:On CNKI,SinoMed,Wanfang data,VIP database,the Cochrane Library,Web of Science,PubMed,Embase databases,the related literature was searched by computer to retrieve randomised controlled trials of treating overlap syndrome by BiPAP.RevMan 5.3 software was used to perform a meta-analysis.Results:A total of 15 studies met the inclusion criteria.The results of the meta-analysis showed that BiPAP could significantly ameliorate the indicators of FEV_(1)/FVC%,pH,PaO_(2),PaCO_(2) and AHI of COPD patients with OSAHS.Conclusion:BiPAP can significantly ameliorate the indicators of FEV_(1)/FVC%,pH,PaO_(2),PaCO_(2) and AHI in COPD patients with OSAHS,and its clinically rational application can significantly ameliorate the prognosis of patients.
