摘要：颞下颌关节紊乱综合征(Temporomandibular Joint Disorer,TMD)发病率逐渐呈上升趋势,以疼痛、下颌运动异常和关节杂音为主要临床表现,给患者带来极大痛苦。通过对近十年来治疗颞下颌关节紊乱综合征有关文献的查阅分析及资料收集,发现有越来越多的手段可治疗TMD,例如针灸治疗、推拿按摩疗法、物理疗法、封闭疗法、药物疗法、心理疗法等。事实上,对于TMD的治疗是一个复杂的综合治疗过程,不同类别、不同患者的治疗方案可能完全不同,因此,在颞下颌关节紊乱的治疗中需注意针对不同的患者进行具体分析,并制订其个人的具体治疗方案。为了达到颞下颌关节紊乱病的最佳治疗目的,给更多的临床医生提供一个对TMD既清晰又合理的治疗思路,从而更好地实践于临床,服务于患者。因此,对TMD治疗手段的收集与整理显得尤为重要。未来寻求疗效佳、经济安全、简易操作的多手段形成的综合疗法应是治疗颞下颌关节紊乱综合征的重点研究方向。因此我们在总结评述的同时,提出近年来治疗颞下颌关节紊乱综合征研究的不足以及今后研究的重点及方向。然而颞下颌关节紊乱综合征临床疗效研究尚需更多严谨的科研设计,大样本、多中心、随机双盲对照试验来验证可靠性。
Temporomandibular joint disorer(TMD),with pain,abnormal mandibular movement and joint murmurs as the main clinical manifestations,has a growing incidence,which brings great pain to patients.Through the literature review and data collection on the treatment of TMJ disorder syndrome in the past ten years,it is found that there are more and more methods to treat TMD,such as acupuncture and moxibustion,massage therapy,physical therapy,occlusion therapy,drug therapy,psychological therapy,etc.In fact,most of the treatment for TMD is a complex and comprehensive treatment process,and the treatment plan may be completely different for different types and patients.Therefore,in the treatment of TMJ disorders,attention should be paid to the specific analysis of different patients and the formulation of their individual specific treatment plan.How to achieve the best purpose of the treatment of temporomandibular joint disorders,irder to provide more clinicians with a clear and reasonable treatment of TMD,so as to practice in the clinic and serve patients.Therefore,the collection and collation of TMD treatment means is particularly important.In the future,it should be the key development direction for the treatment of TMD disorder syndrome to seek the comprehensive therapy with the best curative effect,economic safety and simple operation.Therefore,in summary and review,the deficiency of recent studies on the treatment of TMD and the focus and direction of future studies are proposed,that is,the clinical efficacy study of TMD still needs rigorous scientific research design,large sample,multi-center,randomized double-blind controlled trials to verify the reliability.
