中医外治药物治疗痔术后并发症用药规律及机制探析:基于文献数据挖掘和网络药理学

摘要:目的:探讨中医外治药物治疗痔术后并发症的用药规律及核心药物的可能作用机制。方法:检索中国知网、万方、维普、PubMed、SinoMed等数据库,查找从建库到2022年12月31日之前,发表并证实对于痔术后并发症有确切疗效的文献。使用Excel 2016版进行药物频次及功效统计分析,应用R studio 4.2.1软件分析中医外治药物治疗痔术后的关联规则,应用IBM SPSS Statistics 26.0软件对高频药物进行聚类分析。并使用TCMSP数据库整理高频药物靶点及有效成分,在OMIM、GeneCards、DisGeNET、DrugBank数据库检索术后疼痛疾病靶点,然后将药物与靶点核心、靶点进行GO功能富集分析和KEGG信号通路富集分析。结果:最终纳入中药外治方总计186首,176味中药。用药频次前5的中药依次为黄柏、苦参、大黄、蒲公英、五倍子。痔术后常用外治药物以清热燥湿、活血化瘀药为主;归经主要为肝、肺、胃、脾、大肠经;药性以寒、温、平为主;药味以苦、辛、甘、涩为主,使用频次≥20次的有30味中药,可聚类为5类。高频药物组有效活性成分73个,药物作用靶点225个,疾病作用靶点2339个,两者共有作用靶点171个。高频药物治疗痔术后并发症通路包括PI3K/Akt信号通路、IL-17信号通路、HIF-1信号通路、AGE/RAGE信号通路等。结论:中医外治药物治疗痔术后并发症的功效以清热燥湿、活血止痛为主,兼顾利水渗湿、收涩解毒,临床用药广泛。核心药物治疗痔术后并发症具有多靶点、多途径的特点,其作用机制可能与影响炎症因子、神经递质受体活性、免疫调节等相关。
Objective:To investigate the medication rule of external medicine in the treatment of hemorrhoids postopera-tive complications and the possible mechanism of action of core drugs.Methods:The literatures from CNKI,WanFang,VIP,PubMed,SinoMed and other self-established databases to December 31,2022 were retrieved and analyzed.Excel 2016 version was used for statistical analysis of drug frequency and efficacy,R studio 4.2.1 software was used to ana-lyze the association rules of TCM external treatment drugs after hemorrhoids surgery,IBM SPSS Statistics 26.O high-frequency drug cluster analysis.High-frequency drug targets and active ingredients were sorted out using TCMSP data-base,and postoperative pain disease targets were searched in OMIM,GeneCards,DisGeNET and DrugBank databases.Then GO functional enrichment analysis and KEGG signaling pathway enrichment analysis were conducted between drugs and target cores and targets.Results:186 TCM external remedies and 176 TCM were included in this study.The top 5 most frequently used Chinese herbs were Huangbai,Kushen,rhubarb,dandelion and Gallnut.After hemorrhoids operation,the commonly used external drugs are mainly clearing heat and drying dampness,promoting blood circulation and removing blood stasis.The main channels are liver,lung,stomach,spleen and large intestine.The medicinal proper-ties are mainly cold,warm and flat.Bitter,bitter,sweet and astringent were the main medicinal tastes.There were 30 TCM with frequency≥20 times,which could be clustered into 5 categories.There were 73 active ingredients,225 drug targets and 2339 disease targets in the high-frequency drug group,with 171 targets in total.The pathways for the treatment of postoperative complications of hemorrhoids with high-frequency drugs include PI3K/Akt signaling path-way,IL/17 signaling pathway,HIF-1 signaling pathway,AGE/RAGE signaling pathway,etc.Conclusion:The efficacy of traditional Chinese medicine in the treatment of hemorrhoids postoperative complications is mainly to clear heat and dry dampness,promote blood circulation and relieve pain,and take into account the benefits of water and dampness,as-tringent and detoxification.Core drugs have the characteristics of multi-target and multi-pathway effects in the treat-ment of postoperative complications of hemorrhoids,and their mechanism of action may be related to the influence of in-flammatory factors,neurotransmitter receptor activity and immune regulation.
