摘要：目的:探讨口服阿立哌唑和奥氮平快速控制精神分裂症患者激越症状的相对效果及安全性。方法:163例精神分裂症住院患者,采用随机数字表法分为奥氮平组(n=85,口服20 mg/d)或阿立哌唑组(n=78,口服15 mg/d,治疗第2天起逐渐增加至30 mg/d),试验为期5 d,期间,按需使用右佐匹克隆(剂量1~3 mg/d)治疗失眠。采用阳性和阴性症状量表(PANSS)和简明精神病量表(BPRS)评估有效性,检测两组患者的空腹血糖、甘油三酯和催乳素水平。结果:两组患者PANSS-EC评分和BPRS-阳性症状因子评分均较基线期低(P<0.01),但组间差异无统计学意义(P>0.05)。治疗后奥氮平组空腹血糖和甘油三酯水平高于阿立哌唑组(P<0.01),血清催乳素水平奥氮平组高于阿立哌唑组,差异有统计学意义(P<0.01)。结论:在首次药物治疗的前5 d,奥氮平和阿立哌唑对精神分裂症患者激越症状疗效相似,但奥氮平组血糖和血脂升高更多。
Objective： To observe the relative efficacy and safety of olanzapine and aripiprazole in Rapidly control of agitated symptom in schizophrenia. Methods： 163 patients were entered in this study,all patients were randomly divided into olanzapine group（ n = 85,20 mg/d） and aripiprazolegroup（ n = 78,15 mg/d,increasing to 30 mg/d from second days.）. The trial last for 5 days,during which,Eszopiclone was also given as needed（ 1-3 mg/d）. The efficacy was assess using PANSS and BPRS,fasting blood glucose,triglyceride and prolactin were also assessed. Results： The PANSS-EC and BPRS-positive symptom scores were lower than baseline in both group（ P〉0. 01）,but there was no statistically significant difference between the two groups（ P〈0. 05）. After treatment,fasting glucose and triglycerides increased more significantly in olanzapine-treated patients（ P〈0. 01）. The prolactin in olanzapine group was higher than that in aripiprazole group,the difference was statisthcally significant（ P〈0. 01）. Conclusion： During the first 5 days of randomized treatment,olanzapine and aripiprazole displayed similar efficacy in control of agitated symptom,But the blood sugar and blood lipid increase more in olanzapine group.
