摘要：目的以李景华、郭维琴、曹玉山教授为代表,探索名老中医治疗高血压病的共性、差异及形成原因。方法运用Excel、R语言、Gephi等软件对FangNet平台中李景华、郭维琴、曹玉山教授治疗高血压病的病例进行频数分析、热图制作、聚类分析及复杂网络拓扑结构分析。结果频数分析及热图结果显示,头晕、头痛、失眠为三位教授高血压病患者的共有症状;平肝、潜阳为三位教授共有治法;药物以补虚药最多,其次是活血化瘀药、平肝息风药、清热药等;性味归经以寒性、甘味、肝经为主。复杂网络拓扑结构分析和聚类分析结果显示,三位教授用药存在差异,李景华用药分2个社群,聚成3类;郭维琴用药分4个社群,聚为7类;曹玉山用药分3个社群,聚为8类。结论名老中医对高血压病的治疗以《黄帝内经》象思维作为基础,因此存在共性。治疗重在平肝潜阳,实者兼以息风清热、活血化痰,虚者以益气养阴为要。用药多寒温同用,甘苦辛并举,以肝经为主。受学脉传承、临床经验等因素的影响,名老中医治疗上存在差异。李景华善用柴胡剂组方,清利肝胆、泻胃降逆;郭维琴肝脾同治,强调活血化瘀;曹玉山重在平肝潜阳,滋养肝肾,祛瘀化痰。
Objective To explore the commonalities and differences in the treatment of hypertension by famous Chinese medicine practitioners and the reasons for their formation represented by Professors Li Jinghua,Guo Weiqin and Cao Yushan.Methods Data processing,frequency analysis,heat map production,cluster analysis and complex network topology analysis were performed on cases of hypertension treated by Professors Li Jinghua,Guo Weiqin and Cao Yushan in FangNet platform using the software of Excel,R language and Gephi.Results The frequency analysis and heat map results showed that dizziness,headache and insomnia were the common symptoms of hypertensive patients among the three professors;pacifying the liver and subduing Yang were the common treatments among the three physicians;drugs were most often used to nourish deficiency,followed by activating blood circulation and resolving blood stasis,pacifying the liver and calming the wind,and clearing heat;The sexual taste and ascription were mainly cold,sweet and liver meridians.The results of complex network topology analysis and cluster analysis showed that there were differences in the medicines used by the three professors;Li Jinghua’s medicines were divided into 2 communities and clustered into 3 categories;Guo Weiqin’s medicines were divided into 4 communities and clustered into 7 categories;Cao Yushan’s medicines were divided into 3 communities and clustered into 8 categories.Conclusion The treatment of hypertension by famous veteran TCM practitioners is based on Xiang thinking of the“Inner Canon of Huangdi”,so there are commonalities in treatment.The treatment of hypertension focuses on pacifying the liver and submerging the Yang,while those who are in actuality also use to calm the wind and clear heat,activate blood and resolve phlegm,and those who are in deficiency use to benefit Qi and nourish Yin.The medicinal properties are mostly cold and warm.The medicine used is mostly cold and warm,sweet,bitter and pungent.Medicinal ingredients are mostly sweet,bitter and pungent.Meridian distribution is mainly liver meridian.There are differences in the treatment of famous veteran TCM practitioners such as academic lineage transmission and clinical experience.Li Jinghua makes good use of the formula of Chai Hu agent to clear the liver and gallbladder,diarrhea the stomach and subdue rebellion.Guo Weiqin treats the liver and spleen together,emphasizing activating blood circulation and resolving blood stasis.Cao Yushan emphasizes pacifying the liver and subduing Yang,nourishing the liver and kidney,resolving blood stasis and phlegm.
