摘要：目的调查甘肃省天水市低龄老年人养老模式选择意愿及其影响因素,为推进天水市老年人养老事业提供参考。方法2023年7—8月采用分层抽样方法,对天水市秦州区、武山县和张家川回族自治县60~69岁老年人进行问卷调查;采用χ^(2)检验、秩和检验和无序多分类logistics回归分析低龄老年人养老模式选择的差异及影响因素;以Andersen模型为理论框架,分析先决因素、促能因素和需求因素对天水市低龄老年人养老模式选择意愿的影响。结果纳入低龄老年人952人,823人(86.45%)选择家庭养老,70人(7.35%)选择社区养老,59人(6.20%)选择机构养老;无序多分类logistic回归结果显示,年收入(0~2000元:OR=0.22,95%CI:0.06~0.79;>2000~10000元:OR=0.28,95%CI:0.08~0.93)、子女数量(2个:OR=2.30,95%CI:1.11~4.79)、对所在地卫生服务中心满意度(一般:OR=2.60,95%CI:1.26~5.36)、是否愿意入住养老机构(愿意:OR=4.66,95%CI:2.57~8.46)以及是否感到孤独(经常:OR=5.07,95%CI:1.54~16.63)影响低龄老年人的社区养老意愿。养老观念(自己:OR=0.34,95%CI:0.13~0.85;子女:OR=0.27,95%CI:0.10~0.69)、对医养结合养老模式了解程度(从未听说:OR=0.18,95%CI:0.04~0.78)、子女数量(0个:OR=7.31,95%CI:0.97~55.05)、是否愿意入住养老机构(暂不确定:OR=3.74,95%CI:1.29~10.82;愿意:OR=9.80,95%CI:4.21~22.83)以及是否感到孤独(经常:OR=5.61,95%CI:1.70~18.58)是影响天水市低龄老年人机构养老意愿的因素。结论天水市低龄老年人养老选择以家庭养老为主,受先决因素、促能因素和需求因素的不同程度影响,建议天水市相关部门根据实际情况,关注低龄老年人多元化养老需求,提高养老服务质量。
Objective To investigate the willingness of low-age elderly individuals in Tianshui City to choose different retirement models and their influencing factors,providing references for promoting the elderly care services in Tianshui City.Methods A stratified sampling method was used to conduct a questionnaire survey among low-age elderly people in Qinzhou District,Wushan County,and Zhangjiachuan Hui Autonomous County in Tianshui City from July to August 2023.Chi?square tests and ordinal logistic regression analyses were used to analyze the differences and influencing factors.The Andersen model served as a theoretical framework to explore the impacts of predisposing,enabling,and need factors on the willingness of elderly individuals in Tianshui City to choose retirement models.Results Among the 952 low-age elderly people,823(86.45%)preferred family care,70(7.35%)opted for community-based care,and 59 cases(6.20%)selected institutional care.The results of the ordinal logistic regression showed that annual income(0-2000 CNY:OR=0.22,95%CI:0.06-0.79,>2000-10000 CNY:OR=0.28,95%CI:0.08-0.93),number of children(2:OR=2.30,95%CI:1.11-4.79),satisfaction with the local health centres(average:OR=2.60,95%CI:1.26-5.36),willingness to live in a nursing home(willing:OR=4.66,95%CI:2.57-8.46),and feelings of loneliness(often:OR=5.07,95%CI:1.54-16.63)significantly influenced the willingness for community care.Factors affecting the willingness for institutional care included attitudes towards retirement(self:OR=0.34,95%CI:0.13-0.85;children:OR=0.27,95%CI:0.10-0.69),understanding of the integrated healthcare and nursing model(never heard of:OR=0.18,95%CI:0.04-0.78),number of children(0:OR=7.31,95%CI:0.97-55.05),willingness to live in a nursing home(undecided:OR=3.74,95%CI:1.29-10.82;willing:OR=9.80,95%CI:4.21-22.83),and feelings of loneliness(often:OR=5.61,95%CI:1.70-18.58).Conclusions Family care is the predominant choice for elderly individuals in Tianshui City,influenced by various predisposing,enabling,and need factors.It is recommended that relevant authorities in Tianshui City pay attention to the diverse retirement needs of elderly individuals and improve the quality of elderly care services based on actual conditions.
