摘要：目的对比经尿道前列腺等离子切除术(PRP)与经尿道前列腺电切术(TURP)对良性前列腺增生术后患者性功能的影响。方法采用随机双盲前瞻性对照的试验方法,将140例良性前列腺增生患者按照手术方式分为PRP组和TURP组,两组各70例,记录术前、术后6、12月患者的国际前列腺症状评分表(IPSS)评分和国际勃起功能指数评分简表(IIEF-5)评分,观察术后12月阴茎勃起功能障碍、精量增加、精量减少、射精疼痛及逆行射精的发生情况。结果 PRP组退出10例,TURP组退出8例,PRP组患者术后6、12月的IIEF-5评分均较TURP组高(P<0.05),阴茎勃起功能障碍的发生率较TURP组低(P<0.05);在IPSS评分以及精量增加、精量减少、射精疼痛及逆行射精的发生率方面两组差异无统计学意义。结论 PRP与TURP相比,能够降低患者术后阴茎勃起功能障碍的发生,对患者术后性功能的影响小,具有良好的临床应用前景。
Objective To compare the postoperative sexual function of plasniakinetic resection of the prostate(PRP) with transurethral resection of the prostate(TURP) for benign prostatic hyperplasia(BPH).Methods 140 patients with BPH were equally divided into the PRP group and TURP group.The international prostate symptom score(IPSS),international index of erectile function(IIEF-5) score,the incidence of erectile dysfunction,ejaculation increase,ejaculation reduce,painful ejaculation and retrograde ejaculation were observed.Results The IIEF-5 scores in the sixth month and twelve month postoperatively were higher in the PRP group than in the TURP group and the incidence of erectile dysfunction was higher in the TURP group.There were no significant differences between the two groups in the IPSS score,the incidence of ejaculation increase,ejaculation reduction,painful ejaculation and retrograde ejaculation.Conclusion Feweradverse effects on sexual function are caused by PRP,and PRP may be a promising operation for BPH.
