摘要：子宫内膜息肉(endometrial polyp,EP)是子宫局部间质过度生长被覆良性腺体上皮,是一种常见的妇科疾病,临床常表现为异常子宫出血。目前EP的治疗以手术为主,手术治疗可以切除异位病灶,缓解临床症状,提高育龄期妇女妊娠率,但手术后高复发率成为临床有待解决的一个难点,预防或降低术后复发率也是需要深入探究的一个新方向。一些危险因素的存在可能会影响患者的预后,主要包括年龄、息肉数量、激素水平、体重指数(body mass index,BMI)、合并妇科疾病、分子生物学因素、基因相关、随访时间,而术后及时规范使用口服短效避孕药是EP术后复发的保护因素。EP术后患者需要进行长期管理,而术后不同的治疗方法各有优势和适用场景,可以提高患者预后。本文将对EP切除术后复发的危险因素及术后治疗方法进行综述,为预防、治疗EP术后复发提供依据。
Endometrial polyp(EP)is a local stromal overgrowth covered by benign glandular epithelium in the uterus,which is a common gynecological disease.It is often clinically manifested as abnormal uterine bleeding.Currently,the treatment of EP is mainly based on surgery,which can remove the ectopic lesion,relieve clinical symptoms and improve the pregnancy rate of women of reproductive age,but the high recurrence rate after surgery has become a difficult clinical problem to be solved,and the prevention or reduction of postoperative recurrence rate is also a new direction that needs to be explored in depth.The presence of some risk factors may affect the patients'prognosis,mainly including age,number of polyps,hormone level,body mass index(BMI),comorbid gynecological diseases,molecular biological factors,gene-related,follow-up time,and timely and standardized use of oral short-acting contraceptives after surgery is a protective factor for postoperative recurrence after EP.Postoperative patients after EP need to be managed in the long term,and the different postoperative treatments each have their own advantages and applicable scenarios that can improve the patients'prognosis.In this article,the risk factors and postoperative treatments for postoperative recurrence after EP resection are reviewed to provide a basis for the prevention and treatment of postoperative recurrence of EP.
