摘要：动眼神经又被称为第Ⅲ对颅神经,支配提上睑肌和4条眼外肌。由各种病因引起的动眼神经及其支配组织功能障碍称为动眼神经麻痹。动眼神经麻痹常为病变同侧眼的上直肌、下直肌、内直肌及下斜肌中一条或数条肌肉麻痹,出现眼位改变,常表现为眼肌麻痹、上睑下垂、眼球运动受限、瞳孔扩大、瞳孔对光反射迟钝或消失等症状。该病发病率不高,病因却复杂多样,多由其他颅内外疾病引起,如颅内的动脉瘤、炎症、肿瘤、血管病变及颅脑外伤等,除此以外,其他系统疾病如糖尿病、上呼吸道感染、艾滋病等也可造成动眼神经麻痹。临床尚无明确诊疗策略,治疗较为困难。目前西医对该病的治疗方法包括手术干预、营养神经药物或糖皮质激素等,但效果均不理想。中医治疗动眼神经麻痹的方法多种多样,有单纯针刺、针刺联合其他方法(如艾灸、穴位注射、火针点刺、电梅花针、眼睑康复训练等)、单纯穴位注射、中西医结合和其他治疗方法。针刺联合其他手段不仅丰富了动眼神经麻痹的治疗手段,还可以增强疗效,具有不良反应小、操作简单、创伤小、患者接受度高、安全性高和应用范围广泛等优点。文章介绍1例动眼神经麻痹病例,通过中西医结合治疗后取得较好的疗效,现总结出来,供临床借鉴。
The oculomotor nerve,also known as the third pair of cranial nerves,innervates the levator palpebralis muscle and four extraocular muscles.The dysfunction of the oculomotor nerve and its innervating tissues resulting from a variety of diseases is known as oculomotor nerve paralysis.The symptoms of oculomotor nerve paralysis include paralysis of one or several muscles of the superior rectus,inferior rectus and internal rectus,and inferior oblique muscles on the same side of the eye,and changes in eye position,which are often characterized by paralysis of the eye muscles,drooping of the upper eyelid,restriction of eye movement,pupil dilation,and delayed or absent pupil reflexes to the light.The incidence of this disease is not so high,but the etiology is complicated and varied,mostly caused by other intracra-nial and extracranial diseases,such as intracranial aneurysm,inflammation,tumors,vascular lesions,and craniocerebral trauma.In addition,other systemic diseases,such as diabetes mellitus,upper respiratory tract infections,and AIDS,can also cause oculomotor nerve paralysis.There is no clear clinical diagnosis and treatment strategy,and the treatment is difficult.At present,the treatment methods of Western medicine for this disease include surgical intervention,neurotrophic drugs or glucocorticoids,etc.,but the effect is not ideal.There are a variety of TCM treatment methods for oculomotor nerve paralysis,including simple acupuncture,acupuncture combined with other methods(such as moxibustion,acupoint injec-tion,fire needle acupuncture,electro-plum-blossom acupuncture,eyelid rehabilitation training),simple acupoint injection,the integrative medicine and other treatment methods.Acupuncture combined with other treatments not only enriches the treatment of oculomotor nerve paralysis,but also enhances the efficacy,it has the advantages of few adverse reactions,simple operation,little trauma,high patient acceptance,high safety,and a wide range of applications treated in the integrative medicine.This paper introduces one case of oculomotor nerve paralysis treated in the integra-tion medicine,which has achieved better therapeutic effects,It is summarized now for the clinical reference.
