摘要：糖尿病肾病是糖尿病最常见的微血管并发症,血流动力学的改变贯穿整个糖尿病肾病病变过程,长期高血糖引起的高灌注,严重影响肾小球滤过功能,患者肾脏血流受阻、肾功能降低、肾组织纤维化是疾病进一步恶化的主要原因。糖尿病肾病继发于糖尿病,中医认为其病程日久,在气阴两虚证的基础上,阴虚燥热血液凝炼加之气虚无力推动血液运行,瘀血阻滞肾络,这一变化与血流动力学改变不谋而合。因此文章将中医瘀血阻络理论与血流动力学改变相类比,以期从现代医学角度阐释“瘀血阻络”理论在糖尿病肾病发生发展过程中的作用,为糖尿病肾病临床诊疗提供思路。
Diabetic nephropathy is the most common micro-vascular complication of diabetes.Hemodynamic changes run through the entire process of diabetic nephropathy.Hyperperfusion caused by long-term hyperglycemia seriously affects glomerular filtration function.Renal blood flow is blocked and renal function Decrease and renal tissue fibrosis are the main reasons for the further deterioration of the disease.Diabetic nephropathy is secondary to diabetes.Traditional Chinese medicine believes that the course of the disease is prolonged.On the basis of deficiency of both Qi and Yin,blood condenses due to Yin deficiency,dryness and heat,combined with the weakness of Qi to promote blood circulation,and blood stasis blocks the kidney collaterals.This change is related to blood flow.The dynamics change coincidentally.Therefore,this article compares the theory of blood stasis blocking collaterals with hemodynamic changes in order to explain the role of“blood stasis blocking collaterals”theory in the development of diabetic nephropathy from the perspective of modern medicine,and provide ideas for clinical diagnosis and treatment of diabetic nephropathy.
