摘要：目的探讨尿毒症皮肤瘙痒患者采用中药复方经皮肤透析+血液灌流治疗的临床效果。方法便利选取2020年2月—2023年5月武山县人民医院收治的52例尿毒症皮肤瘙痒患者为研究对象,依据投掷硬币法分为对照组和研究组。对照组(26例)采用常规治疗+血液透析+血液灌流,研究组(26例)在参照组基础上增加中药复方经皮肤透析治疗。比较两组治疗总有效率、小分子毒素、瘙痒VAG评分、视觉模拟评分法(Visual Analogue Scale,VAS)评分、改良Duo评分、甲状旁腺激素(Parathyroid Hormone,PTH)、钙磷乘积以及β2-微球蛋白。结果研究组治疗总有效率(96.15%)高于对照组(76.92%),差异有统计学意义(χ^(2)=4.127,P<0.05)。治疗后,研究组血肌酐水平、尿素氮水平、瘙痒VAG评分、VAS评分、改良Duo评分、PTH水平、钙磷乘积以及β2-微球蛋白均低于对照组,差异有统计学意义(P均<0.05)。结论尿毒症皮肤瘙痒患者采用中药复方经皮肤透析+血液灌流方法治疗,可明显提升患者的治疗效果,显著改善小分子毒素、瘙痒症状、疼痛症状以及中大分子毒素。
Objective To investigate the clinical effect of treatment of uremic pruritus with Chinese medi?cine compound by skin dialysis and hemoperfusion.Methods 52 patients with uremic pruritchy skin admitted to Wushan County People's Hospital from February 2020 to May 2023 were conveniently selected as the study objects and divided into the control group and the study group according to coin throwing method.The control group(26 cases)received conventional treatment+hemodialysis+hemperfusion,and the study group(26 cases)added traditional Chinese medicine compound to the reference group for skin dialysis treatment.The to?tal response rate,small molecule toxin,pruritis VAG Score,Visual Analogue Scale(VAS),modified Duo score,parathyroid hormone(PTH),calcium and phosphorus product andβ2-microglobulin were compared between the two groups.Results The total effective rate of the study group(96.15%)was higher than that of the control group(76.92%),and the difference was statistically significant(χ^(2)=4.127,P<0.05).After treatment,the serum creatinine level,urea nitrogen level,pruritus VAG score,VAS score,modified Duo score,PTH level,calciumphosphorus product andβ2-microglobulin in the study group were lower than those in the control group,and the differences were statistically significant(all P<0.05).Conclusion The treatment of patients with uremic pru?ritus with Chinese herbal compound via skin dialysis+hemoporfusion can significantly improve the therapeutic effect of patients, and significantly improve the symptoms of small molecular toxins, pruritus symptoms, painsymptoms and medium and large molecular toxins.
