摘要：目的 :探讨哮喘反复发作的机理。方法 :把现代医学研究哮喘的焦点“气道高反应性”概念 ,纳入《金匮要略》的理论中探讨 ,提出哮喘发病机制的全新理论假说 :“哮喘反复发作的根本原因是 :‘脾肺阴虚为本 ,痰湿淤阻气道为标’”。病机是 :“肺脏阴血不足 ,受邪所迫 ,冲脉应激 ,携肺气上逆” ,产生非特异性免疫反应 ,哮喘炎症启动 ,也即“气道高反应性”状态。依据以上假说 ,组成“喘康丸” ,在缓解期治疗。 ( 1)治疗组 ( 5 1例 ) ,设定观察气阴两虚型患者在服喘康丸 60天后和急性发作时的 :临床综合疗效、症状积分值、体征积分值、治疗性疗效评价、对血清IgE ,EC的影响。 ( 2 )以桂龙咳喘宁胶囊作临床对照 ( 5 1例 )。结果 :①喘康丸可明显降低哮喘患者急性发作时的症状、体征 ,总有效率为 90 0 % ,对照组 68% ,两组比较 ,差异有显著性 (P <0 0 5 ) ;②治疗组在下调IgE、EC指标的作用方面也优于对照组 (P <0 0 5 )。结论 :喘康丸可增强肌体抗病能力 ,补益脾肺 ,有效降低“气道高反应性” ,预防哮喘反复发作。
Objection:To explore the mechanism of preventing the repeated altack of asthma(气阴两虚型) during alleviation period were randomly divided into a group treated with “chuankangwan” and a group treated with the capsules of “GuiLongKeChuanNing” on average.Sixty days constituted one course.Then observe dinical comprehensive therapeutic effects,integration of symptoms,integration of signs,assessment of therapeutic effects,effects of blood IgE and EC when bronchinal asthma altacks acutely.Results:1.“ChuankangWan” could significantly relieve the patients' symptoms and signs of acute attack.The Total effective rate of treatment group were 90.0%,and that of efficacy between the two groups (P<0.05). 2.The treatment group was superior to the control group in decreasing the indexes of InE and EC. Condusion:“ChuanKangWan” could enhance the ability of resistance against illness and relieve symptoms and signs of patients' acute attack.These manifested that “ChuanKangWan” could effectively decrease “airway high vesponse” and prevent repeated attack of bronchial asthma.
