摘要:Aims: Mild cognitive impairment (MCI) is one of the chronic complications of type 1 diabetes (T1DM). Alteration in amino acid metabolism is commonly observed in diseases associated with cognitive impairment, but its role in type 1 diabetes-associated mild cognitive impairment (T1DM-MCI) remains unclear. We aimed to explore the risk factors for T1DM-MCI and identify potential metabolites that can help determine the absence or presence of MCI in T1DM. Materials: This cross-sectional study enrolled 119 participants, comprising 39 with T1DM-MCI, 48 with T1DM and normal cognition (T1DM-NC), and 32 healthy controls. MCI was defined as a Montreal Cognitive Assessment (MoCA) score less than 26. Serum amino acid levels were analyzed using ultra-performance liquid chromatography-tandem mass spectrometry. Correlation and receiver operating characteristic (ROC) analyses were also conducted. Results: The mean age of participants was 36 years, and 44% of the participants were female. Among patients with T1DM, the median disease duration was 7 (range: 3-15) years. Logistic regression analysis revealed that older age (odds ratio [OR] = 1.060; P = 0.008) and lower educational level (OR = 0.851; P = 0.038) were significantly associated with the presence of T1DM-MCI. In addition, compared to the T1DM-NC group, seven metabolites were significantly downregulated in the T1DM-MCI group. L-Valine distinguished T1DM-MCI from T1DM-NC with moderate accuracy (sensitivity: 72.7%, specificity: 75.7%, area under the curve: 0.745, P < 0.001). Conclusions: Our results revealed that in adults with T1DM, MCI may be associated with alterations in amino acid metabolism, providing new insights into the pathogenesis of T1DM-MCI.
