摘要：目的观察归芪白术方辅助治疗胃癌患者的临床研究。方法选择甘肃省武威肿瘤医院就诊的50例胃癌患者,随机分为对照组和试验组各25例。选取体检结果正常的50人为健康对照组。对照组采用西医常规治疗,试验组在西医常规治疗基础上加用归芪白术方治疗,每日1剂(用药剂量保密),分2次服用,每服用6 d、停用1 d。试验组患者在每次化疗治疗的第2周开始服用中药。3个化疗周期为1个疗程。于治疗前及疗程完成后,用酶联免疫吸附法测定血清白细胞介素4(IL-4)、干扰素γ(IFN-γ)、肿瘤坏死因子α(TNF-α)的含量;用试剂盒法检测胃癌患者血清中癌胚抗原(CEA)及糖类抗原72-4(CA72-4)、糖类抗原19-9(CA19-9)的含量。并进行《Kamofsky》评分和美国东部肿瘤协作组(ECOG)评分。结果治疗后,对照组的白细胞介素4(IL-4)、干扰素γ(INF-γ)、肿瘤坏死因子α(TNF-α)和癌胚抗原(CEA)及糖类抗原72-4(CA72-4)、糖类抗原19-9(CA19-9)分别为(52.49±1.48),(16.38±1.14),(234.52±24.09)pg·m L^(-1)和[2.79(0.84,5.31)]ng·m L^(-1)及[2.81(2.12,4.62)],[4.60(2.39,9.72)]U·m L^(-1);试验组的这6个指标分别为(49.95±1.48),(19.85±2.04),(199.80±22.21)pg·m L^(-1)和[0.89(0.76,1.37)]ng·m L^(-1)及[1.87(1.24,3.13)],[2.66(1.05,5.89)]U·m L^(-1)。与治疗前比较,2组差异均有统计学意义(均P<0.05);2组间比较差异也均有统计学意义(均P<0.05)。结论归芪白术方辅助治疗对胃癌患者的血清肿瘤标志物、免疫炎性因子及生活质量具有明显改善作用。
Objective To observe the effect of the auxiliary treatment with Gui Qi Bai Zhu fang in the treatment of patients with gastric cancer.Methods Fifty gastric cancer patients after chemotherapy who met the inclusion criteria in Wuwei city were randomly divided into control group and treatment group,each group has 25 persons. And 50 healthy people as healthy control group was selected. The control group was treated by routine western medicine,the treatment group was treated by Gui Qi Bai Zhu fang based on routine western medicine treatment. The daily dose of Gui Qi Bai Zhu fang was one（ the dosage was secrect）,twice a day,taking for 6 days,stopping for 1 day. Gui Qi Bai Zhu fang was taken in the second week of chemotherapy in the treatment group. Three cycles of chemotherapy were 1 courses of treatment. The content of the interleukin-4（ IL-4）,interferon-gamma（ IFN-γ）,tumor necrosis factor-α（ TNF-α） were detected by enzyme linked mmunosorbent assay beforeand after treatment,Content of carcinoembryonic antigen（ CEA）,carbohydrate antigen 72-4 and（ CA724） and carbohydrate antigen 19-9（ CA19-9） were determined by reagent box method. The score of ＂Kamofsky＂and Eastern Cooperative Oncology Group（ ECOG） was made before and after treatment,respectively. Results After treatment,IL-4,IFN-γ,TNF-α,CEA,CA724,CA19-9 in the control group were（ 52. 49 ± 1. 48）,（ 16. 38 ± 1. 14）,（ 234. 52 ± 24. 09） pg·mL^（-1）and [2. 79（ 0. 84,5. 31） ]ng·mL^（-1）and [2. 81（ 2. 12,4. 62） ],[4. 60（ 2. 39,9. 72） ]U·m L^（-1）; the factors in treatment group were（ 49. 95 ± 1. 48）,（ 19. 85 ± 2. 04）,（ 200. 46 ± 23. 64） pg·m L^（-1）and[0. 89（ 0. 76,1. 37） ]ng·m L^（-1）and [1. 87（ 1. 24,3. 13） ],[2. 66（ 1. 05,5. 89） ]U·m L^（-1） respectively. There had significant difference with those before treatment（ all P〈0. 05）. The IL-4,INF-γ,TNF-α,CA72-4,CA19-9 and CEA had significant difference between the control group and the treatment group after treatment（ all P〈0. 05）.Conclusion The auxiliary treatment with Gui Qi Bai Zhu fang can obviously improve serum tumor marker,the expression of proinflammatory cytokines and quality of life in patients with gastric cancer.
