摘要：目的分析2017—2022年甘肃省14个市州中医类医疗卫生资源配置公平性,探讨影响资源配置水平的主要因素,为甘肃省中医类医疗卫生资源优化配置提供参考。方法选取中医类医疗卫生机构数、卫生技术人员数及床位数作为关键指标,通过熵权法进行归一化处理,结合地理面积和人口规模,运用泰尔指数进行分析;并采用面板时间固定效应模型,引入人均GDP、人口密度等变量,探讨影响甘肃省中医药资源配置的深层因素。结果甘肃省中医类医疗卫生机构、床位、卫生技术人员的年均增长率分别为-1.59%,7.11%,5.17%;熵权法后的资源配置结果显示,陇中地区、河西地区、陇东地区、陇东南地区、民族地区年均增长率分别为10.25%,6.43%,8.58%,4.40%,3.84%,5个地区的中医类卫生资源年均增长率均为正值;影响因素分析中地区人均生产总值、财政自主度、医疗费用财政支出、城镇居民人均医疗保健支出是主要影响因素(P<0.05)。结论甘肃省中医类医疗卫生资源配置整体水平呈增长趋势,但总量仍不足,需进一步加强各地区的中医药资源配置力度;区域内和区域间差异依然存在,应加强各个地区及地区间的中医类医疗卫生资源配置经验共享;此外,还需加大政府投入,积极宣传中医药。
Objective To analyze the fairness of the allocation of traditional Chinese medicine(TCM)medical and health resources in 14 prefecture-level cities of Gansu Province from 2017 to 2022,and explore the main factors influencing the level of resource allocation,providing reference for optimizing the allocation of TCM medical and health resources in Gansu Province.Methods The key indicators of TCM medical and health resources,including the number of TCM medical and health institutions,the number of TCM healthcare professionals,and the number of TCM beds,were selected,and the entropy weighting method was used for normalization.Combined with geographic area and population size,the Thiel index was used for analysis.A panel time fixed-effect model was used to introduce per capita GDP,population density,etc.to explore the deep-level factors affecting the allocation of TCM resources in Gansu Province.Results The annual average growth rates of TCM medical and health institutions,healthcare professionals,and beds in Gansu Province were-1.59%,7.11% and 5.17%,respectively.The resource allocation results after entropy weighting showed that the annual average growth rates of TCM health resources in the Longzhong region,the Hexi region,the Longdong region,the Longdong-nan region,and the ethnic minority areas were 10.25%,6.43%,8.58%,4.40% and 3.84%,respectively.The annual average growth rates of TCM health resources in all five regions were positive.The main influencing factors identified by the factor analysis were per capita GDP,fiscal autonomy,government expenditure on medical fees,and urban residents'per capita medical and health expenditures(P<0.05).Conclusion The overall level of TCM medical and health resource allocation in Gansu Province is increasing,but the total is still insufficient,and efforts should be made to strengthen the allocation of TCM resources in various regions.Regional and interregional disparities still exist,and experience sharing in the allocation of TCM medical and health resources among regions and regions should be strengthened.In addition,the government should increase its investment and actively publicize traditional Chinese medicine.
