摘要:非酒精性脂肪性肝病(non-alcoholic fatty liver disease,NAFLD)是肝脏代谢紊乱的信号,也是肝硬化、肝癌等的预警疾病。目前NAFLD研究集中于炎症性肠病(inflammatory bowel disease,IBD)导致的炎症或肥胖对肝脏的影响,IBD存在的肠道低度炎证已被证明与NAFLD有关。而基于IBD相关性营养不良患者人数的持续上升,本文通过回顾近几年国内外有关IBD相关性营养不良的研究进展与机制,整合有关IBD相关性营养不良影响NAFLD发病的核心原理。进一步从缺乏的各类营养素来阐述营养不良,并介绍肌少症与营养不良双重负担(dual burden of malnutrition,DBM)对NAFLD的影响,旨在为中西医防治NAFLD提供新思路。
Non-alcoholic fatty liver disease(NAFLD)is a manifestation of hepatic metabolic dysfunction and serves as an early warning indicator for severe liver conditions such as cirrhosis and hepatocellular carcinoma.Current research on NAFLD primarily focuses on the hepatic impacts of inflammation induced by inflammatory bowel disease(IBD)and obesity.The persistent low-grade intestinal inflammation characteristic of IBD has been conclusively demonstrated to correlate with NAFLD pathogenesis.Given the persistently increasing prevalence of IBD-associated malnutrition,this study synthesized recent domestic and international research advances by systematically reviewing the pathomechanisms of IBDrelated malnutrition and elucidating its core regulatory pathways underlying NAFLD pathogenesis.Furthermore,this review elucidated malnutrition through the lens of specific nutrient deficiencies,and examined the compounded effects of sarcopenia coupled with the dual burden of malnutrition(DBM)on NAFLD progression.The findings ultimately aim to establish a multidimensional analytical framework for developing integrated traditional Chinese and Western medicine strategies targeting NAFLD pathogenesis.
