摘要：目的观察热补针法联合药物铺灸法治疗类风湿关节炎(RA)临床疗效。方法采用随机数字表法将90例RA患者随机分为热补针法组、药物铺灸组及热补针法+药物铺灸组,各30例。热补针法组取关元、足三里和累及关节局部腧穴,予热补针法,每日1次;药物铺灸组取大椎至腰俞区及两旁的华佗夹脊穴,涂擦鲜姜汁及何氏自拟风湿痹痛散,并施以长蛇灸,隔日1次;热补针法+药物铺灸组治疗方法同以上2组。3组均连续治疗15 d。观察3组治疗前后中医症状积分、视觉模拟量表评分(VAS)评分及血清红细胞沉降率(ESR)、类风湿因子(RF)、C反应蛋白(CRP)水平,比较3组临床疗效。结果与本组治疗前比较,3组治疗后中医症状积分、VAS评分及血清ESR、RF、CRP水平均明显下降(P<0.05);3组治疗后比较,热补针法+药物铺灸组上述指标较其他2组改善更为明显(P<0.05)。热补针法+药物铺灸组总有效率为93.3%,明显优于热补针法组(83.3%)和药物铺灸组(80.0%),差异有统计学意义(P<0.05)。结论热补针法联合药物铺灸疗法治疗RA临床疗效满意,可明显减轻RA患者疼痛症状,降低生化指标,抑制免疫反应。
Objective To observe the clinical efficacy of heat reinforcing needling method combined with TCM moxibustion for rheumatoid arthritis(RA).Methods Totally 90 patients were randomly divided into heat reinforcing needling method group,TCM moxibustion group,and heat reinforcing needling method and TCM moxibustion group according to random number table method,with 30 cases in each group.Heat reinforcing needling method group was given acupoints with Guanyuan(RN4),Zusanli(ST36)and joint local acupoints by using heat reinforcing needling method,once a day;TCM moxibustion group was given acupoints with Dazhui(GV 14)to Yaoshu(GV 2)area,rubbed with ginger juice and HE’s self-made Fengshi Bitong Powder,and applied Changshe moxibustion,once every other day;The treatment for heat reinforcing needling method and TCM moxibustion group was the same as the above two groups.The treatment for the three groups lasted for 15 d.TCM symptom scores,the visual analogue scale(VAS),levels of erythrocyte sedimention rate(ESR),rheumatoid factor(RF)and C-reactive protein(CRP)were observed before and after treatment in the patients of all the three groups.Clinical efficacy was compared.Results Compared with before treatment,the TCM symptom scores,VAS score and serum ESR,RF and CRP levels of the three groups significantly decreased after treatment(P<0.05).After treatment,the above indexes of heat reinforcing needling method and TCM moxibustion group were more obvious than those of the other two groups(P<0.05).The total effective rate of heat reinforcing needling method and TCM moxibustion group was 93.3%,which was significantly better than that of heat reinforcing needling method group(83.3%)and TCM moxibustion group(80.0%),with statistical significance(P<0.05).Conclusion Heat reinforcing needling method combined with TCM moxibustion for RA achieved satisfying clinical efficacy,which can reduce pain symptoms and biochemical indicators in patients with RA,and suppress immune response.
