摘要：硬膜外纤维化是椎板切除术后患者常见的并发症之一,在很大程度上会导致术区组织层之间的粘连和临近神经根的牵拉,引发慢性腰痛和下肢放射痛,其不仅影响了患者的生活质量,而且也降低了手术的治疗效果。目前,临床防治硬膜外纤维化的主要手段有药物治疗、手术治疗及生物抗粘连材料等,但都未获得满意的临床效果。中药活性成分因其成分天然且易提取、副作用小、价廉等优势,具有广泛的应用前景和研究价值。研究发现,中药活性成分具有防治硬膜外纤维化形成的功效,主要通过抑制炎症、提高抗氧化能力及自噬、调节促/抗凋亡蛋白表达、降低成纤维细胞的增殖、分化及迁移能力,减少新血管生成和细胞外基质过度沉积等发挥作用。本文通过检索近5年来国内外有关中药防治硬膜外纤维化的研究文献予以阐述,以期为后续的基础研究提供思路,也为中药运用于临床提供有证可循的研究证据及参考,进一步推动中医药的发展,为临床治疗提供更多的选择和新方法。
Epidural fibrosis is one of the common complications in patients after laminectomy.To a large extent,epidural fibrosis can lead to adhesion between the tissue layers of the operative area and the pulling of adjacent nerve roots,resulting in chronic back pain and radiating pain in lower limbs,which not only affects the quality of life of patients but also reduces the therapeutic effect of the surgery.At present,the main methods of clinical prevention and treatment of epidural fibrosis include drug therapy,surgery,and biological anti-adhesion materials,but they have not achieved satisfactory clinical results.The active ingredients in traditional Chinese medicine(TCM)have wide application prospects and research value because of their advantages such as natural ingredients,easy extraction,low side effect,and low price.It has been found that the active ingredients in TCM can prevent and treat epidural fibrosis by inhibiting inflammation,improving antioxidant capacity and autophagy,regulating pro-/anti-apoptotic protein levels,inhibiting the proliferation,dfferentiation,and migration of fibroblasts,and reducing angio genesis and excessive deposition of extracellular matrix.This paper searched the Chinese and foreign research literature on the prevention and treatment of epidural fibrosis with TCM in the past five years to provide ideas for the follow-up basic research and offer evidence-based research evidence and reference for the application of TCM in clinical practice,so as to further promote the development of TCM and put forward more options and new methods for clinical treatment.
