摘要:对于骨质疏松性椎体压缩骨折(OVCF)的治疗,椎体成形术已成为临床上治疗该病的常用方法,早期使用椎体成形术对椎体进行有效撑开,防止了椎体的进一步塌陷和脊柱力学失稳,减少了卧床并发症,提高了患者的生活质量,具有较好的临床疗效。同时,进一步明确椎体压缩骨折发生的生物力学特点和机制对该病的防治具有重要临床价值,随着计算机软件开发和应用的飞速发展,有限元分析(FEA)在骨科生物力学研究中的应用已经非常成熟。FEA因其具有数字化建模、仿真度较高及运算精准等优势,在椎体成形术治疗OVCF的生物力学研究中应用日趋成熟,并且取得了一定的研究成果。文章通过综述近年来FEA在OVCF生物力学研究中的应用情况,重点分析了椎体成形术治疗OVCF术前、术后以及椎体本身及相邻椎体的生物力学特性变化规律的FEA成果,从力学机制角度对该病的防治策略提供思路,以进一步指导临床实践。
For the treatment of osteoporotic vertebral compression fractures(OVCF)of vertebroplasty has become a commonly used method for the treatment of the disease,clinical early use of vertebroplasty to effectively open of vertebral body,to prevent further collapse of vertebral body and the instability of the spine mechanics,reduced the laid-in-bed complications,improve the patient’s quality of life,has good clinical curative effect.At the same time,further defining the biomechanical characteristics and mechanism of vertebral compression fracture has important clinical value for the prevention and treatment of this disease.With the rapid development of computer software development and application,the application of finite element analysis in orthopedic biomechanics research has been very mature.Due to its advantages of digital modeling,high degree of simulation and accurate calculation,finite element analysis has been increasingly mature in the biomechanical research of vertebroplasty for the treatment of OVCF,and certain research results have been achieved.This article through studies in recent years,the finite element analysis in OVCF in biomechanical research,the application of the key for the treatment of vertebroplasty of OVCF in preoperative,postoperative and vertebral body itself and the adjacent vertebral body biomechanics changes.From the perspective of mechanical mechanism,the prevention and treatment strategy of the disease is provided to further guide clinical practice.
